Where cables which are installed in hazardous areas introduce the risk of fire or explosion in the event of electrical fault in such areas, precautions shall be to the satisfaction of the administration.
Multiple temperature gauge electrical cable connections on the number 1 and 2 cargo pumps are loose fitting and partially exposed creating an ignition hazard in the cargo pump room. Affected electrical connections and pumps shall be isloated and not used during transfers until proven safe.

Resolution: Electric cables of upper and lower mechanical seal/bearing temperature sensor on No. 1, 2 cargo pumps in pump room were disconnected and refitted with rubber packing in cable gland in accordance with the recommendation on maker’s drawing and found sat.

The venting system shall be provided with devices to prevent the passage of flame into the cargo tanks.
Multiple outer flame screens (lower of the 2 screesn) on the cargo thank vent oulets are holed, inadequately preventing the passage of flame. Crew replaced screens on tanks 2 and 5 containing cargo, in order to conduct cargo operations. All other tank vent screens were not verified as replaces by [name withheld] 74(04 ED) CH II-2, REG 4.5.3.3
