Where the main source of electrical power is necessary for propulsion and steering of the ship, the system shall be so arranged that electrical supply to equipment necessary for propulsion and steering and to ensure safety of the ship will be maintained or immediately restored in the case of loss of any one of the generators in service. Vessel experienced a complete loss of electrical power due to a failure of #3 generator. No alarms gave prior indication of abnormal conditions and #1 generator failed to take the load while running in parallel and tripped off.

Before the ships leave port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Vessel reported leaky hydraulic cylinder on free-fall lifeboat davit was unable to be repaired in port due to hot work constraints. Report from certificate issuing authority indicated free-fall launching capability of the lifeboat is fully functional and davit can be used in extreme emergency.

12109- The carriage of dangerous goods in solid form in bulk shall be in compliance with the relative provisions of the IMSBC Code, as defined in eg VI/1-1.1. IMSBC Code 1.2.2 where a solid bullk cargo is specifically listed in appendix 1 to this Code, it shall be transported in accordance with the provisions in its schedule in addition to the provisions in sections 1 to 10 and 11.1.1 of this code. Continuous mechanical ventilation shall be conducted during the voyage for the cargo spaces carrying this cargo. Prior to loading, the valve shall be checked and the valve lid and seat lapped to a fine finish, as necassary. After re-assembly, the valve shall be locked and shut and a notice shall be placed adjacent to the valve warning against opening without the master's permission. Schedule provision requirements (closing valve and ventilation) described were not conducted prior to of during the transport of Ferrosilicon, dangerous cargo. Vessel requires corrective action plan from company with concurrence from administration. SOLAS 74 (a) 2014 cons VII/7.4, ISMBC Code 1.2.2. 17a
