With the town pumps simultaneously delivering water through the nozzles specified in paragraph 2.3.3, with the quantity of water as specified in paragraph 2.1.3, through any adjacent hydrants, the following minimum pressures shall be maintained at all hydrants: for cargo ships, 6000 gross tonnage and upwards: 0.27N/mm^2. Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed rescue boat was unable to start for a period of 25 minutes, in addition, no cooling water was observed. 74 SOLAS 2014 cons., III/20.2 17ac

On all ships, within the machinery spaces, emergency escape breathing devices shall be situated ready for use at easily visible places, which can be reached quickly and easily at any time in the event of fire. The location of emergency escape breathing devices shall take into account the layout of the machinery space and the number of persons normally working in the space. Bulk carriers shall be fitted with water level detectors in each cargo hold, giving audible and visual alarms. PSCO observed system abnormal fault on water ingress alarm system for #1 cargo hold. 74 SOLAS 2014 cons., XII/12.1.1 17ac

Following installation, the master, owner, operator, agent, or person in charge of the vessel subject to this subpart must properly maintain the BWMS in accordance with all manufacturer specifications. PSCO observed two inoperable suction valves that were being manually bypassed in order to operate the BWMS. Request class to attest to current configuration of system. 33 CFR 151.2025(a)(1) 16ac

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. In meeting these requirements, the company should ensure that inspections are held at appropriate intervals and records of these activities are maintained. In reviewing Annex I of the vessel's on board technical manual, PSCO received objective evidence that nearly 100% of the maintenance items were beyond the specified maintenance interval. Each operation described in paragraph 2 of this regulation shall be fully recorded without delay in the Oil Record Book Part I, so that all entries in the bok appropriate to that operation are completed. PSCO observed multiple entries in the Oil [name withheld] indicating that required operations are not being accurately recorded; with emphasis on entries regarding the Waste Oil Tank and Incinerator Waste Oil Tank between 03Apr2022 and 24Apr2022. Oil record book entries regarding the collection, transfer, and disposal of bilge and sludge between March 2022 and April 2022 do not align with onboard sounding records described in deficiency 03, or crew statements regarding onboard operations.

In a ship which oil fuel is used, the arrangements for the storage, distribution, and utilization of the oil fuel shall be such as to ensure the safety of the ship and persons onboard. PSCO observed fluctuations in fuel oil overflow tank levels. Crew reported fuel oil is leaking into fuel day tanks, causing them to overflow into fuel oil overflow tank.

The Company should ensure that the [Environmental-Protection] policy is implemented and maintained at all levls of the organization, both ship-based and shore-based. PSCO observed Envronment Management System Manual, Chapter 03, Page 2 of 37 which states, "A separate register daily IOPP tank soundings register TE39 is supplied to maintain daily tank soundings. The register is to be filled with permanent ink...The readings of this register and the ORB quantities should match accurately." Crew was unable to provide any records that register TE39 had evern been filled out, and PSCO observed that a digital spreadsheet is being used in its place since February 2022. The physical condition and proper storage of mooring lines and ropes to ensure that they are free of damage and defects and are not exposed to conditions which could lead to their deterioration and eventual failure. PSCO observed a deterioated mooring line. ILO-147/PWSA 46USC70002; 60ac

In a ship in which oil fuel is used, the arrangements for the storage, distribution and utilization of the oil fuel shall be such as to ensure the safety of the ship and persons on board. PSCO observed fuel oil accumulation and leaks from fuel delivery valves in the galleries on all three generators. 74SOLAS(09)II-2/4.2.2, 17ac

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations. PSCO observed two soft patches on port side bunker lines. 74SOLAS(14)I/11a, 17ac

Except as provided in paragraph 3 of this regulation, any ship of 10,000 gross tonnage and above shall be fitted with oil filtering equipment complying with paragraph 7 of this regulation. Vessel reported oily water separator inoperable. MARPOL I/14.2, 17ac

74SOLAS (09) II-2/9.1.3 The purpose of this regulation is to contain a fire in the space of orgin. For this purpose, the following functional requirements shall be met: the fire integrity of the divisions shall be maintained at openings and penetrations. A class A boundary in the crews laundry has unapproved penetrations for a door tie-back. 40a.c.
