Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Also, Posters or signs shall be provided on or in the vicinity of survival craft and their launching controls shall illustrate the purpose of controls and the procedures for operating the appliance and give relevant instructions and warnings and be easily seen under emergency lighting conditions. • The lower guard for the forward starboard liferaft was stuck in place and would prevent it from being launched in that position. • The liferaft launching procedures (port and stbd embarkation decks) not posted under e-lighting. All items of lifeboat equipment shall be as small as possible and shall be packed in a suitable and compact form. Except where otherwise stated, the normal equipment of every lifeboat shall consist of anti-sickness medicine sufficient for at least 48 hours. The lifeboat did not have enough seasickness medicine for 30 passengers. The lifeboat had 240 tablets and required 360 tablets. Also, there were no dosage instructions for the medication. The Company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. All ships of 500 gross tonnage and above engaged on international voyages to carry portable atmosphere testing equipment/s on board the ship from 1 July 2016. Ensure that on board portable testing equipment is suitable for the depth of the tank being certified safe for entry. Current on board testing unit only have approximately 10 feet of hose which is inadequate to reach the bottom of the deepest void. This is to be completed prior to confined space entry. SOALS XI-1/7

Make report to ABS and provide a route cause analysis as to why the main journal bearings between cylinder 3 and 4 has become dislodged on the main engine. SOLAS I-11(a)

Oil fuel lines shall be arranged far apart from hot surfaces, electrical installations or other sources of ignition shall be screened or otherwise suitably protected to avoid oil spray or oil leakage onto the sources of ignition. Tape covering connection points on the fuel oil line on the generators were worn or missing. SOLAS 04 II-2/4.2.2.5.3

For every seafarer on call, such as when a machinery space is unattended, the seafarer must have adequate compensatory rest period if the normal period of rest is disturbed by call-outs to work. Reviewed STCW work rest hours when an alarm was answered while the machinery space was unattended. 2nd A/E logged work, but did not receive adequate rest. 46 CFR 15.1111(h)

The maintenance program of a 46 CFR 62.50-20(h) must include a checkoff list to make sure routine daily maintenance has been performed, fire and flooding hazards have been minimized, and plant status is suit- able for unattended operation. Completion of this checkoff list must be logged before leaving the plant unattended. Provide maintenance program check-off list [See 46 CFR 62.50-20 (h)] utilized for unattended plant operation. Provide log entries prior to leaving engine room unattended. Cite: 46 CFR 62.50-30 (j)

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. A power strip was utilized to recharge the flash lights in the port side fire locker. Cite: SOLAS (2004) II-1 / Reg 45.1.3

Remote fuel shut offs for Op zone 5 quick-closing valves were not able to close when actuated.

Ship was unable to successfully test the emergency fire pump as requested during the inspection due to mechanical failure of the pump.

Required vessel to provide documentation from attending Class Surveyor attesting to satisfactory alteration made to the drain piping to the oil settling tank in the oil purifier space (see photos in Documents for further information).

Vessel could not prove proper operation of the emergency fire pump.

Provide fire rating certificate for newly installed fire doors for the hospital, linnen locker and engine room.

The vessel is granted interim approval to operator as a periodically unattended Machinery Space. During this period the vessel will be reevaluated at the next scheduled dry-dock on reliable operation and maintenance.

Prove proper operation of the refrigeration alarms.

Remove storage from the bottom of the deck in main staircase tower.

Repair deck drains throughout the vessel.

Provide a contigency plan for missing smoke detector in the paint locker.

Install an intrinsically safe smoke detector in the paint locker.

Make repairs to the weathertight doors on the A deck.

Prove transitional power with the EDG to automatically start and supply the required output within 45 sec.
