Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Both port and starboard lifeboat exhaust pipes were completely disintegrated resulting in exhaust gases leaking into the boat. Permanently repair both exhaust pipes to the satisfaction of the servicing technician, Class, and Coast Guard. 74 SOLAS (2020) III/20.2

Efficient guard rails or bulwarks shall be fitted on all exposed parts of the freeboard and superstructure decks. The height of the bulwarks or guard rails shall be at least 1 metre (39½ inches) from the deck. Damaged guard rails were found amid ships between the cargo holds and aft of the superstructure. Permanently repair damaged guard rails in above locations prior to conducting cargo operations in the vicinity. ICLL 66/88 Annex I Regulation 25.2

A main electric lighting system shall provide illumination throughout those parts of the ship normally accessible to and used by passengers or crew. Marine Inspector observed several dozen regular and emergency lighting fixtures inoperable. 74SOLAS (2004) II-1/41.2.1

A duplicate set of fire control plans or a booklet containing such plans shall be permanently stored in a prominently marked weathertight enclosure outside of the deckhouse for the assistance of shore-side firefighting personnel. The fire control plans located on both the port and starboard side weather deck and inside the house were last revised in 2007 and have no class stamp or indication that they are up-to-date. 74SOLAS (2004) II-2/15.2.4.2

Where A class divisions are penetrated, such penetrations shall be tested in accordance with the Fire Test Procedures Code. Such penetrations shall be suitably insulated by extension at the same level of the division. Marine Inspector observed multiple uninsulated cable penetrations in the cable trunks throughout accommodations. 74SOLAS (2004) II-2/9.3.1

Terminations and joints in all conductors shall be so made as to retain the original electrical, mechanical, flame-retarding and, where necessary, fire resisting properties of the cable. Marine Inspector observed a junction box in the galley for the deep-fat fryer broken, wrapped in electrical tape to preserve the connection and to prevent user from being exposed to live electrical conductor. 74SOLAS (2004) II-1/45.5.6

All life-saving appliances and arrangements shall comply with the applicable requirements of the Code. Except where otherwise state, the normal equipment of every lifeboat shall consist of: a food ration as described in 4.1.5.1.18 totaling not less than 10,000 kJ for each person the lifeboat is permitted to accommodate; these rations shall be kept in an airtight packaging. Marine Inspector observed 2 rations in port lifeboat and 3 rations in the starboard lifeboat that are no longer airtight. 74SOLAS (2020) III/34 LSA Code 4.4.8.12

The following plans must be submitted to the Coast Guard for approval in accordance with §§ 50.20-5 and 50.20-10 of this chapter: a. Design Verification and Periodic Safety test procedures described in subpart 61.40 of this subchapter. Vessel could not provide an authorized copy of Periodic Safety Test Procedures. 46 CFR 62.20-1

Electrical equipment and wiring shall not be fitted in enclosed cargo spaces or vehicles spaces unless it is essential for operational purposes in the opinion of the Administration. However, if electrical equipment is fitted in such spaces, it shall be of a certified safe type for use in the dangerous environments to which it may be exposed unless it is possible to completed isolate the electrical system. The explosion proof lighting in Cargo Hold1/ Bay 2, leading 1DWBT Center, were identified with cracked lenses. SOLAS (2009) II-2/ Reg 19.3.2

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Galley mushroom vent housing on lifeboat deck was found deteriorated. Also, vent screen was clogged reducing air flow. 74 SOLAS (20) 1/11 50A

All access openings in bulkheads at ends of enclosed superstructures shall be fitted with doors of steel or other equivalent material, permanently and strongly attached to the bulkhead, and framed, stiffened and fitted so that the whole structure is of equivalent strength to the unpierced bulkhead and weather tight when closed. Marine inspector discovered port side superstructure access doors on upper and lower lifeboat deck unable to close naturally. ICLL (88 Proto) 12 50A

Electrical installations shall be such that the safety of passengers, crew and ship from electrical hazards will be ensured. Starboard aft quarter floodlight fixture bracket is broken and is currently not supported. 74 SOLAS (20)II-1/40.1.3 50A

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. Marine inspector discovered three (03) fuel oil purifier control boxes broken and needing replacement. 74 SOLAS (04) II-1/45.1.3 50A

The ship’s designs and arrangements, as a whole, complies with the relevant prescriptive requirements in parts B, C, D, E or G. Verify the installed structural fire protection complies with the SOLAS requirements to convert the Cleaning Gear Locker on E-deck to a small laundry facility.

Accommodation and service spaces and control stations of cargo ships shall be protected by a fixed fire detection and fire alarm system and/or an automatic sprinkler, fire detection and fire alarm system as follows depending on a protection method adopted in accordance with regulation 9.2.3.1. Verify the vessel’s equipped detection/firefighting method appropriately protects the Cleaning Gear Locker on E-Deck for conversion into a laundry installation.

General arrangement plans and booklets shall be kept up to date; any alterations thereto shall be recorded as soon as practicable. Modify the fire control plan to reflect the presence of the Small Laundry Facility on E-Deck in the space originally labeled as Cleaning Gear Locker.

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: structural fire protection including fire resisting divisions, and protection of openings and penetrations in these divisions. Repair the fire stop material in the cable ways of the Cable Trunk located on A-Deck.

Cylinders must always be secured and when not in use they must be stowed in a rack in an upright position with the valve protection cap in place. Oxygen and acetylene bottles in the lockers are not secured and missing caps.

Stairways which penetrate more than a single deck shall be surrounded by at least "A-0" class divisions and be protected by self-closing doors at all levels. Port side Exterior door on the navigation deck in the stairway main vertical zone was missing cover over the fire hose port.

The company and the ship shall comply with the requirements of the ISM Code. For the purpose of this regulation, the requirements of the Code shall be treated as mandatory. The Company should ensure that all personnel involved in the Company's safety management system have an adequate understanding of relevant rules, regulations, codes, and guidelines [ISM 6.4]. ...Each lifeboat shall be launched and maneuvered in the water by its assigned operating crew [SOLAS III/19.3.3.3]. Maersk Line Limited Lifeboat, Rescue Boat and Davit Launched Life Raft Drills - Precautions on Launching and Recovery Document ID: 07-01-02-06.900 states "lifeboats and rescue boats must be lowered with only the minimum number of persons on board necessary to operate the boat." [name withheld] refused to conduct lifeboat and rescue boat drills in accordance with the companies procedures and the applicable regulatory requirements during the inspection. Additionally it was stated that when conducting lifeboat and rescue boat drills, they do not lower the lifeboat with an operating crew on board and have no intention on conducting drills in this manner. An internal SMS audit focusing on lifesaving procedures is required within 60 days.

Rescue boat did not start, engineer repaired loose wire, cleared on the spot.

Immersion suits not stowed as to the plan. Vessel Master unaware of stowage of suits on deck with labels, vessel plan has suits stowed in locker. Crew confused on where to find the jumbo immersion suits.

33 CFR 164.37(a) Each vessel of 10,000 GT or more must have, in addition to the radar system under 164.35a, a second marine radar system that operates indepedently of the first. Vessel's S-band radar is inoperable.

Provide EIAPP certificate for main engine and auxiliaries. In the Interim, the vessel is authorized to sail on IAPP issued by the Recognized Organization (RO) which is to be accompanied by the EIAPP supplement issued by LR on behalf of Hong Kong. Verify EIAPP to the satisfaction of a USCG Marine Inspector or LR Class Surveyor.
