07126- Means shall be provided to control leaks of flammable liquids. PSCO observed fuel accumulation on top of the MGO fuel filter in the purifier room. Filter was installed incorrectly after cleaning and was leaking from the filter on top.

Resolution: New gasket was fitted to cylinder head NO.5. Lube oil pressure gauge joint and cylinder head cover was dismantled, cleaner and tightened.
1410 - Propulsion Main Engine - Means shall be provided to limit the accumulation of flammable vapors. PSCO's received tech report for marine casualty. Provide tech report and request to replace MDE Scavenge air cooler and clean fuel/carbon deposits on piston, cylinder liner, and scavenge air space. Whenever an accident occurs to a ship or a defect is discovered, the master or owner of the ship shall report to the administration, the nominated surveyor or recognized organization responsible for issuing the relevant certificate who shall cause investigations to be initiated. PSCO discovered technicians working on the MDE turbo drive to turbo failure while at sea.
