In addition to the emergency power sources required under part 112 of this chapter, each self-propelled vessel and each mobile offshore drilling unit must have at least two electric generating sources.
Vessel experienced a #1 service generator shutdown resulting in a loss of power and subsequent loss of steering while transiting outbound the Galveston Shipping Channel.
Diagnose power failure and complete permanent repairs to the satisfaction of vessel's Classification Society.
*Vessel is authorized a one-time transit from current offshore position to AET Galveston Marine Base.*

Vessel reported a sheared drive belt on the #1 service generator resulting in an unplanned generator shutdown.
Review generator drive belt manufacturer's maintenance and inspections requirements and evaluate current vessel maintenance procedures for compliance with manufacturer's specifications and / or Class Rules.
Action required: 50 - Rectify deficiencies w/in 30 days

Vital systems that are remotely controlled must be provided with an effective primary control system.
When the pneumatic operated fuel shut off valves were actuated, the starboard #2 day tank shut off valve did not close due to the valve being manually over-ridden. This is not the first occurrence of this happening within the AET fleet. Company is required to develop procedures prior to getting underway to ensure all automatic fuel shut off valves are in the automatic position to ensure capability of pneumatic operation.

Sufficient illumination must be provided by the emergency lighting source under emergency conditions to effect damage control procedures and to provide for safe egress from each space.
03 Emergency lights found extinguished in steering space, steering corridor, and port escape trunk in main engine space. E-Lights in escape trunks Port and STBD were not marked with "E" designating as Emergency lighting. Lights bulbs were replaced and marked prior to MI departure.
