ENCOUNTER

A flammable vapor detection system required by 46 CFR 182.410(c) must be located in the lowest part of a space containing a fuel tank above the expected bilge water level. 46 CFR 182.480

A vessel must carry a first-aid kit approved under ISO 18813:2006(e) paragraph 4.12. Update first aid kit. 46 CFR 184.710

At each inspection for certification, the operator shall be prepared to conduct test of its fire protection equipment, including testing of alarms and ventilation shutdowns. 46 CFR 176.810

The OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Remove paint from fuel tank mounting brackets for annual visual inspections.

Each distribution panel and switchboard must be in as dry a location as practicable, adequately ventilated, and protected from falling debris and dripping or splashing water. Install splash shield on distribution panel at stern. 46 CFR 183.330

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that: (1) The pump is listed by an independent laboratory as meeting the requirements in UL 1113. On-board bilge pumps are not UL1113 listed. 46CFR182.520

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency... New crew members have not completed familiarization and drills. 46CFR185.420 ***To be cleared by attending Marine Inspector***

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible. Onboard charts are not current. 46CFR184.420(a)(1)

Except on a vessel limited to daytime operations, the compass must be illuminated. Compass light on-board is not illuminated. Restricted to daytime operations. 46CFR184.402(c)

The Certificate of Inspection and any SOLAS Certificates must be posted under glass or other suitable transparent material, such that all pages are visible. All pages of the COI are not displayed. 46CFR176.302

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels. Operator could not provide evidence of consortium enrollment. 46CFR 16.230(a)

The servicing facility must issue a certificate to the liferaft owner or owner's agent for each liferaft it services. Owner/operator did not have servicing certificate issued by the servicing facility. 46CFR176.302

For fixed-gas fire extinguishing systems...The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. O/O could not provide servicing records for fixed gas suppression system. 46CFR176.810(b)(2)

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”, unless such markings are deemed unnecessary by the cognizant [name withheld] on-board is fading. 46CFR185.606

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. (1) Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.110. (2) Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. (3) Each floating waterlight installed after March 11, 1997, on a vessel carrying only one ring buoy, must be attached to the lanyard with a corrosion-resistant clip. The clip must have a strength of at least 22.7 kilograms (50 pounds), and allow the waterlight to be quickly disconnected from the ring life buoy. Vessel float light bracket is broken so the float light is not supported / protected while the vessel is underway. Properly repair or replace the float light bracket. 46 CFR 180.70

IAW with D17 Instruction 16714.1A dtd 5 Mar 2007. Vessel must have on board a FCC Type Accepted Cat 1, 406 MHz EPIRB, installed to automatically float free and activate. Vessel's EPIRB's hydrostatic release is expired. Renew expired hydrostatic release. 46 CFR 180.64

Vessel is required to have onboard bilge pump arrangements iaw 46 CFR 182.520(a). Vessel is using submersible electric bilge pumps to meet the requirement. Vessel's aft bilge pump was not working when tested. Repair aft bilge pump. 46 CFR 182.520

Each survival craft must be secured to the vessel by a painter with a float free link permanently attached to the vessel. Vessels painter was not connected to the wink link. Connect IBA painter line to the weak link on the hydrostatic release. 46 CFR 180.130(a)

Replace expired hydrostatic release on IBA.

Replace expired EPIRB batteries.

Provide proof of enrollment in a drug and alcohol testing program, including a list of covered employees.

Properly label all ADULT and CHILD PFD storage locations with type and quantity.

Provide onboard updated marine charts.

Properly stencil vessel's name and hailing port onto the vessels stern.

Properly repair minor hairline cracks at the base of the cabin side doors. Aprox 2" - 3" inches in length. Monitor until repaired and notify the local OCMI if the condition worsens. Due by the vessels 2017 hull/ise exam.

Ensure all employees are pre-employment drug screened and enrolled in company's DAPI program.

Properly install a marine grade valve in the fwd void electrical submersible bilge overboard piping.

Properly repair the audible alarm located at the helm station on the vessels fire boy system. All other aspects of the system are functional/working properly.

Replace vsl's 2 gallon fire buckets with 2.5 gallon fire buckets.

Vessel reduced to 26 total persons due to missing two type I PFD's.

Properly renew vsl's expired EPIRB hydrostatic release.

Properly repair broken hinges for vessels anchor line and chain stowage void hatch cover.

Vessel restricted to 1000' fm shore unless master has a valid FCC Marine Radio Operators Permit.

Have onboard non expired alcohol test strips due to remote ops.

Properly label emerbency exits.

Provide 3'-5' line for vsl's float light.
