AT APPROXIMATELY 1000 ON 28APR21, PSCO'S ATTENDED THE VESSEL TO CONDUCT AN ISPS EXAM DUE TO THE REPORT AN ABSCONDER WHO GAINED ILLEGAL ENTRY INTO THE U.S. ONCE ON ABOARD PSCO'S CONFIRMED THAT CREW CONDUCTED AN EXTENSIVE SEARCH OF THE VESSEL WITH NEGATIVE RESULTS OF THE CREW MEMBER.
EXTERNAL VERIFICATION OF ISSC IS REQUIRED PRIOR TO RELEASE OF DETENTION. (CONTINUED....) VERIFICATION SHOULD FOCUS ON CONTROLLING ACCESS TO THE SHIP AND NOTIFICATION PROCEDURES.

The safety management system should include procedures ensuring that non-conformities, accidents, and hazardous situations are reported to the company, investigated and analysed with the objective of improving safety and pollution prevention.
On 17 October 2020 the Yangtze Dawn experienced an accident (fire) in their main engine room which created a hazardous situation necessitating the use of primary fire-fighting equipment. The Coast Guard received reports that one of the systems used, the hyper-mist, mayhave been less effective than desired, which required subsequent use of the fixed CO2 system to combat the fire.
Submit evidence to [email withheld] detailing how this accident/ hazardous situation was reported to the company. investigated and analyzed (with particular focus on primary fire-fighting systems, includiong the efficacy of the hyper-mist) whith the objective of improving safety, including the implementaion of any corrective action and measures intended to prevent recuurence, (as detailed and required in ISM A/9).
