Each vessel vapor connection flange face must have a permanent stud projecting outward that has a 12.7 millimetere diameter and is at least 25.4 millimeters long. It must be located at the top of the flange face, midway between boltholes, and in line with the bolthole pattern. PSCO observed the aft port side vapor collection flange pin pushed back away from the face and the forward port side vapor collection flange pin missing. 46 CFR 39.2001(j) 10c

Residues from tall oil, crude and tall oil pitch were found in the discharge containment under the transfer connections on the stbd and port side. The dishcharge containment required as applicable is in place and periodically drained to provide the required capacities. Ship is to remove all cargoes and cargo residues from contaiment.

Fresh water for the sern launch lifeboat persons was not on board lifeboat before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for imediate use. Ships crew located the fresh water for the lifeboat and placed it inside the lifeboat.
