Main and auxiliary machinery essential for the propulsion and safety of the ship shall be provided with effective means for its operation and control. While conducting mooring operations, the vessel lost means of propulsion.

All electrical apparatus shall be constructed and so installed as not to cause injury when handled or touched in the normal manner. PSCO observed dead-end wires not energized throughout the engine room. Also observed electrical tape on live wires that were not properly housed. 74SOLAS(04) II-1/45.1.3

The arrangements for the storage, distribution and utilization of other flammable oils employed under pressure in power transmission systems, control and activating systems and heating systems shall be such as to ensure the saftey of the ship and persons on board. PSCO observed oil soaked lagging on the main diesel engine. 74SOLAS(01) II-2/15.4

Means shall be provided to contorl leaks of flammable liquids. PSCO observed accumulation of oil by pressure valves. 74SOLAS(04) II-2 Reg 4.1.1.

The following fire protection systems shall br kept in good order so as to ensure their required performance if a fire occurs: structural fire protection, including fire resisting divisions, and protection of openings and penetrations in these divisions. PSCO observed unapproved penetration was made through A/C unit ducting in engin control room. 74SOLAS(04) II-2 Reg 14.2.1.1.

The machinery, boilers, and other pressure vessels associated piping systems and fittings shall be of a design and construction adequate for the service which they are intended and shall be so installed and protected as to reduce to a minimum any danger to the persons on board, due regard being paid to moving part, hot surfaces, and other hazards. PSCO observed excessive leaking on steering gear pump no. 2 during test. 74SOLAS(04)II-2 Reg 26.1.

The activation of any detector or manually operated call point shall initiate a visual and audible fire signal at the control panel and indicating units. PSCO observed three inoperable signal columns at three different locations throughout machinery space; forward engine room, STBD side generator room, and purifier room.

In addition to the requirements contained elsewhere in the present regulations, ships shall be designed, constructed and maintained in compliance with the structural, mechanical and electrical requirements of a classification society which is recognized by the administration in accordance with the provisions of regulation XI/1, or with applicable national standards of the administration which provide an equivalent level of safety. PSC officers observed significant wastage and corrosion on elevated walkways and lashing platforms resulting in a safety hazard. Conditions observed to include but not limited to the elevated walkways and lashing platforms forward of bays 01, 04, 09, 38, and 49. Code 40ab

The company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the relevants rules and regulations established by the company. The technical and operational nature of the deficiencies noted on deficiencies 02, 03, and 04 provide objective evidence of a serious failure of the implementation of the ISM Code. An external audit report satisfactory to the OCMI must be submitted prior to clearance of this deficiency. 74 SOLAS (2020) IX/13.1 ISM Part A 10.1

Fire-extinguishing appliances shall be kept in good order and be ready for immediate use at all times. PSCO observed securing pins installed on three out of 383 vessel's pressure release valves on the fixed CO2 system. The vessel's manual for this system states "The securing pins and the steel wire must always be removed when a system is put into operation." and requires 383 bottles to fight a fire in the vessel E/R. The bottles with the securing pins installed were #8, #50, and #113. With these securing pins installed the required number of CO2 bottles would not have discharged in the case of an emergency. Posing a significant threat to the safety of the vessel and its crew. 74 SOLAS (2001) II-2/21

PSCO observed excessive corrosion on the elevated walkways around each container bay and cracks to the hand rails on the starboard stairs leading to the aft mooring deck. 74 SOLAS (2001) II-1/3-1

In a ship in which oil fuel is used, the arrangements for the storage, distribution and utilization of the oil fuel shall be such as to ensure the safety of the ship and persons on board. PSCO observed excessive F/O leak on cylinder # 14 on the M/E, throughout the purifier room, and auxiliary throughout generator. 74 SOLAS (2001) II-2/15.2

PSCO observed excessive cooling water leak from the high temp M/E jacket water cooler. 17ac

Every steam pipe and every fitting connected there to through which steam may pass shall be so designed, constructed and installed as to withstand the maximum working stresses. PSCO observed steam leak on cut off valve and pressure regulating valve for control steam to LO purifier. 17ac

In a ship in which oil fuel is used, the arrangements for the storage, distribution and utilization of the oil fuel shall be such as to ensure the safety of the ship and persons onboard. PSCO observed excessive F/O leak on #2 HFO purifier feeder pump. 17ac

After any survey of the ship under regulation 10 has been completed, no change shall be made in the structural arrangement covered by the survey, without the sanction of the amdministration. PSCO observed two modified wooden dooors welded to the bulkhead on the 2nd deck. 40ac 74SOLAS (20) I/11 (b)

PSCO observed excessive fresh water leak from freshwater line in passageway leading to the paint locker. 50ac SOLAS II-1/26.1

The examiner observed serveral CO2 cylinders on the fixed exstinguishing system had a considerable amount of rust despite recent service on 08MAY21. Class and Flag were made aware of the USCG observation.

The purpose of this regulation is to maintain and monitor the effectiveness of the fire safety measures the ship is provided with. For this purpose, the following functional requirements shall be met: Fire protection systems and firefighting systems and appliances shall be maintained ready for use. Activated emergency fie pump and tested fire hose on starboard bridge wing and tested fire hose which discharged oil mixed with firefighting water. Examined fire station’s and found additional 02 to be contaminated with oil on deck and 02 contaminated fire stations in the engine room. Also found hole in main fire pipe leaking water into a 55-gallon drum which filled completely in one hour. Lastly, found 06 fire extinguishers overdue for servicing. Vessel needs to create proposal plan for cleaning entire fire main system, a plan for safely disposing of oil from the fire main system and needs to have adequate firefighting measures in place during the cleaning process.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. While conducting engine room exam, found 18 inches of oily water mixture in starboard side aft bilge well underneath shaft seal and in starboard side barge well underneath propeller shaft. See first deficiency for oil found in fire main system.

The safety management system should include procedures ensuring that non-conformities, accidents and hazardous situations are reported to the company, investigated and analyzed with the objective of improving safety and pollution prevention. Master explained that vessel sucked up oil from a sheen in the water while sailing through the Persian Gulf two months ago via the ship’s emergency fire pump. One month later, BOSN reported that the pool “reeked” of a fuel oil smell while attempting to fill the pool. Captain failed to report either instance to the company and was not surprised when fire hose on bridge wing discharged oily water mixture. In addition, galley fixed CO2 extinguisher had not been checked since March 2019. Battery room extinguisher has not been checked since June 2019. Engine room had 04 extinguishers overdue for annual servicing but are marked “ok “ or “good” on monthly servicing tag. Recommend full ISM audit.

THE NAME AND PORT OF REGISTRY OF THE SHIP TO WHICH THE LIFEBOAT BELONGS SHALL BE MARKED ON EACH SIDE OF THE LIFEBOATS BOW IN BLOCK CAPITAL LETTERS OF THE ROMAN ALPHABET. WHILE CONDUCTING THE ABANDON SHIP DRILL, IT WAS OBSERVED THAT NO NAME OF SHIP OR PORT WAS PRESENT ON EITHER OF THE LIFEBOATS.

EACH OPERATION DESCRIBED IN PARAGRAPH 2 OF THIS REGULATION SHALL BE FULLY RECORDED WITHOUT DELAY IN THE OIL RECORD BOOK PART 1, SO THAT ALL ENTRIES IN THE BOOK APPROPRIATE TO THE OPERATION ARE COMPLETED. WHILE REVIEWING OIL RECORD BOOK PART 1, IT WAS OBSERVED THAT THE ENTRY MADE ON 13JULY2019 EXCEEDED OIL FILTERING EQUIPMENT MAXIMUM RATE OF FILTRATION BY 9M3 OVER A PERIOD OF 6 HOURS.

After any survey of the ship under regulations 7, 8, 9, or 10 has been completed, no change shall be made in the structural arrangements, machinery equipment and other items covered by the survey , without the sanction of the administration. Vessel made repairs to the top of port side fuel tank in cargo hold #3 and dod not get classification society approval. USCG requires class report before sailing to next port.

Where it is intended to carry flammable or toxic liquids in enclosed cargo spaces, the bilge pumping system shall be deisgned to ensure against inadvertant pumping of such liquids through machinery space piping or pumps. Fuel oil was observed in the fire main while testing fire hoses on 11SEPT2019. Verify crossover connectionvalve is not leaking when in closed position, preventing fuel oil from inadvertantly entering fire main.
