Ships carrying cargoes for which 15.12, 15.12.1, or 15.12.3 is listed in column o in the table of chapter 17 shall have on board sufficient but nhot less than three complete sets of safety equipment. The PSCO observed 01 of 03 safety equipment had a tear in the face hood seal and an inoperable zipper. 74 SOLAS (24) VII/10.1 IBC 14.2.1

Every ship shall be provided with fire pumps, ifre mains,hydrants and hoses complying as applicable with the requirements of this regulation. The PSCO observed a leak in the firemain piping on the forward cargo seciton next to the number one cargo tank. 74 SOLAS (97) II-2/4.1

Not less than one half of the total number of lifebuoys shall be provided with self-igniting lights complying with the requirements of regulation 31.2. The PSCO observed a life buoy on the centerline adminship with an inoperable self-igniting light. 74 SOLAS (97) III/7.1.3

ELETRICAL INSTALLATIONS SHALL BE SUCH THAT ALL ELETRICAL AUXILIARY SERVICES NECESSARY FOR MAINTAINING THE SHIP IN NORMAL OPERATIONAL AND HABITABLE CONDITIONS WILL BE INSURED WITHOUT RECOURSE TO THE EMERGENCY SORCE OF ELETRICAL POWER. VESSEL EXPERIENCED A LOSS OF POWER AND PROPULSION DURING MOORING EVOLUTIONS.

To discharge ballast water into the waters of the United States, the master, owner, operator, agent or person in charge of a vessel subject to 151.2025 of this subpart must either ensure that the ballast water meets the ballast water discharge standard in 151.2030(a), use an AMS as described in 151.2025(a)(3), or ballast with water from a U.S. public water system, according to the schedule in paragraph (b) of this section. Subject vessel, of keel laid date 19JUN1998 and ballast water capacity of 3117 m3, is past its compliance date per the implementation schedule in 33 CFR151.2035 (b). Currently, vessel’s installed BWTS is inoperative. Vessel is not authorized to discharge untreated ballast into the waters of the U.S. without prior written authorization from the local Captain of the Port under the provisions of 33 CFR 151.2040 (b).

74SOLAS(97 CONS) III/19.2 Before the ship leaves port and at all times during the voyage all life saving appliance shall be in working order and ready for immediate use. Starboard lifeboat sprinkler system unable to protect vessel. Vessel has repair to the starboard bow that is not documented.

74SOLAS(97 CONS) III/46.2 A fire protected lifeboat when water borne shall be capable of protecting the number of persons it is permitted to accomodate when subjected to a contious oil fire that envelopes the life boat for a period of not less than 8 mins. Starboard lifeboat sprinkler system is not in working order and available for immediate use.
