PSCO observed excess hull growth on the stern and port side of the vessel's hull.

While conducting bridge examination PSCO noted a low level alarm for supply tank of hyper mist fire fighting system. (SOLAS 14 II-2/10.4.1.1.3, FSS Code 8.2.3.2.1)

Resolution: PSC Mobile attended vessel and observed a full supply tank for hypermist fire extinguising system satisfying the requirement.

Before the ship leaves port and at all times during the voyage, all life saving appliances shall be in working order and ready for immediate use. While conducting examination of life saving equipment, PSCO noted that the free fall lifeboat was unable to be started. It was determined that the lifeboat was fueled with contaminated fuel. Vessel's crew was able to drain the fuel tank, add fuel, and start lifeboat successfully as witnessed by PSCO. (SOLAS 14 III/20.2)

APPROXIMATELY 100 OUT OF 122 INSTALLED CO2 ACTUATORS WERE FOUND WITH SAFETY PINS IN PLACE, WHICH RENDERED THE CO2 SYSTEM INOPERABLE AS INDICATED IN THE SYSTEM'S OPERATIONS MANUAL - 74 SOLAS(09) II-2/14.2.1.2 - 30ac

OBJECTIVE EVIDENCE DURING AN EXPANDED ISM EXAM REVEALED THAT THE SHIP'S PROCEDURES RELATED TO THE INSPECTION OF INSTALLED CO2 FIRE-FIGHTING SYSTEM DO NOT PROVIDE ENOUGH DETAIL TO ENSURE ADEQUATE INSPECTIONS OF THE CO2 SYSTEM BY THE CREW. IN ADDITION, THE CREW IS NOT KNOWLEDGABLE OF CO2 SYSTEM OPERATIONS MANUAL. IT IS RECOMMENDED THAT AN EXTERNAL THIRD PARTY AUDIT OF THE SMS BE CONDUCTED WITH REGARD TO DEFICIENCIES NO. 01 AND NO. 02. 74 SOLAS (14) IX/ 3.1, ISM CODE PART A 10.1 - 30ac
