No person shall conduct an oil or hazardous material transfer operation unless transfer hoses and loading arms are long enough to allow the vessel to move to the limits of its moorings without placing strain on the hose, loading arm, or transfer piping system. Master stated to PSCO that the bunker piping sheared due to force applied as receiving and delivering vessels separated due to sea conditions, resulting in bunkering product accumulating on deck. 33 CFR 156.120(b)

Vessel must have an installed CG approved BWMS or AMS to discharge ballast water into the waters of the United States. Vessel's BW extension expired on 01JUN19 and a system was not installed. Vessel cannot discharge ballast water into water's of the US unless they employ a method as per 33CFR151.2025(a)(2),(4) and (5). 33CFR151.2035

Resolution: Vessel will need to drydock to make necessary isntallment of equipment, they are still required to hold ballast while in US waters unless employing method per 33CFR151.2025(a)(2), (4) and (5). Should expand examination of BWM.

Resolution: The ship's crew renewed all O-Rings on high pressure fuel lines. After completion of repairs, the main engine was operationally tested with no apparent leaks detected.

06107- Where inerting or padding of cargo tanks is required by the International Bulk Chemical (IBC) Code the vessel shall have an adequate supply of inert gas for use. The crew was unable to operate and test the alarms/indicators required for the installed nitrogen generator system in accorance with the FSS Code. SOLAS 74(14)VII/10 IBC CODE 9.1.3

Resolution: Multiple alarms were tested for the N2 Generator which were found to operating as per manufacturer's specifications.
