PSCO observed the vessel's sewage tank overflowing after reviewing daily sounding records, it was determined that the 4th extinguisher was not taking correct soundings of the tank. 4th engineer recorded in the last sounding that the tank was approximately 18 m^3, about 25% of the capacity. PSCO believes the 4th Engineer was not given proper familiarization with his duties. 74 SOLAS(14) IX/3.1 ISM CODE 6.3
