The company should establish procedures, plans, and instructions, including checklists as appropriate, for key shipboard operations concerning the protection of the environment. The oil record book recordings regarding bilge holding tank soundings as required by MARPOL I/17.4, contain several inaccuracies of tank quantities. The safety management system does not address procedures for identifying or accounting for inaccurate entries.
