Machinery subject to internal pressure including pressure vessels, means shall be provied where practicable to protect against excessive pressure. During engine room exam, Coast Guard examiner could not identify safety relief valves on both air recievers. Provide documentation or current arrangement or proper installation of required safety relief valves. 74 SOLAS (04) II-1/27.2

The company and the ship shall comply with the requirements of the International Safety Management Code... The safety management system should provide for specific measures aimed at promoting the reliability of such equipment or technical systems that are not in continuous use. Inspection reports indicated satisfactory condition and no non-conformity reports, while material condition identified multiple issues. Multiple fire extinguishers were found not inspected for months/not in serviceable condition. Secondary means of communication (phone) in steering gear was found not operating properly, no report made. Fixed foam applicator inoperable due to severe corrosion deterioration, raw water cooling pump(s) leaking excessively, multiple smoke detectors being held together by tape, no report made, and an engine room safety rail was discovered creating a severe falling hazard from the upper engine room. In addition, the crew incorporated an OWS non-approved operating procedure that involved flushing sea water with oily waste to avoid the oil content meter reading above 15 ppm so that the effluent could be pumped directly overboard. Crew informed PSCO this was the only way to get system to operate. An ISM external audit is required prior to departing the port of New York/ New Jersey. 74 SOLAS (09 Consol.) Ch IX Reg 1.3.1 / ISM Code 12.1

The purpose of this regulation is to suppress and swiftly extinguished a fire in the space of origin. Fire Extinguishing appliances shall be readily available. Port State Control Officers identified a fixed foam applicator on the fire control plan located in the overhead of the aux. machinery space behind the main electrical panel in way of the engine control room with severe wastage due to corrosion/no record of maintenance. In this space, the A 60 boundary was compromised, and smoke detectors were functioning, but being held together by tape and not reported. 1974 SOLAS 2009 II-2/ 10.1.2

Hydraulic power-operated steering gear shall be provided with the following: arrangements to maintain the cleanliness of the hydraulic fluid taking into consideration the type and design of the hydraulic system. The PSCO’s observed multiple hydraulic fluid leaks in the steering gear room. The leaks came from the fill system and the high pressure hydraulic lines and left hydraulic fluid to flow down the steering equipment. 74 SOLAS (09 Consol.) CH II-1 Reg. 29.12.1

Fire extinguishers shall be situated ready for use… The PSCO’s observed many portable fire extinguishers not been examined for months thus making them not ready for use. Multiple fire hoses were not in operating condition due to soiling & rot. 74 SOLAS (09 Consol.)CH II-2 Reg. 10.3.2.4

Cables and wiring shall be installed and supported in such a manner as to avoid chafing or other damage. Various cable runs throughout machinery spaces were missing hardware to properly secure overhead wire runs. 74 SOLAS (09 Consol.) CH II-1/Reg 45. 5.5

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. The design shall have regard to materials used in construction, the purpose for which the equipment is intended, the working conditions to which it will be subjected and the environmental conditions on board. All (03) Raw water cooling pumps in the engine room were leaking excessively. Further, #3 generator has excessive fuel oil leaks on the #5 cylinder fuel oil pump. 74 SOLAS (09 Consol.)Ch.II-1 Reg. 26.1

Satisfactory means... shall be provided for the protection of the crew in getting to and from their quarters, the machinery space and all other parts used in the necessary work of the ship." Vessels engine room upper-rail was bent causing a falling hazard. LLC Reg. 25.4

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without without danger to the ship or persons on board.” The PSCO’s examined many different light fixtures that had damaged/broken glow switch starters. PSCO’s examined over 20 different light fixtures in the main machinery space with this condition. The PSCO’s examined multiple smoke detectors that were cracked or temporarily repaired with clear tape. 74 SOLAS (09 Consol) CH. I Reg. 11

Protection of cargo pump rooms in tankers. 02 vertical stairwell lighting fixtures in the cargo pump room were not gas tight. Horizontal fixture in the lower cargo pump room in way of emergency communications booth had a temporary seal on both ends. Gas tight condition could not be verified. 74 SOLAS (09 Cons)Ch.II-2 Reg. 4-5.10.

The following requirements shall govern application of the tables: Tables 9.7 and 9.8 shall apply respectively to the bulkhead and decks separating adjacent spaces." In accordance with table 9.8, control stations adjacent with machinery spaces of category A require A-60 boundaries. The ECR adjacent with the auxiliary space did not have a proper A-60 boundary.

The venting arrangments shall be so designed and operated as to ensure that neither the pressure nor vacuum in cargo tanks shall exceed design parameters. Vessel found with cargo vapors to be venting out vacuum side of pressure/vacuum valve on cargo tanks #3 port, #2,5,7,8 stbd, while tanks are under positive pressure. Vessel is required to repair valves to satisfaction of class and USCG prior to departure and prior to issuance of COC. 74 SOLAS (14) II-2/11.6.1

Condition of ship and its equipment shall be maintained to conform to the present regulations to ensure the ship in all respects will remain fit to proceed to sea without danger to ship/persons on board. The grease fittings on gastight seals on cargo oil pump #1,2,4 and ballast pump found missing or broken. 74 SOLAS (14) I/11

Cables shall enter an explosion protected enclosure only by means of a gland or equivilent device capable of maintaining the integrity of the integrity of the enclosure. P/S bilge high level alarms in the cargo pumproom found with wires to be pulled out of enclosure. 74 SOLAS (14) II-1/45.11, IEC 7.5.1

The inert gas system referred to in paragraph 5.5.3.1 shall be designed, constructed and tested in accordance with FSS code. Deck seal low water level alarm (simulation mode) is not working as designed ( no indication that alarm is working). Crew drained the deck seal water in attempt to simulate low level alarm but could not refill deck seal with water via designed fill line. PSCO also observed multiple cracks in the deck seal sight glass. 74 SOLAS (09) II-2/reg. 4/5.5.3 FSS Code CH 15 Code: 60 a & c

The machinery, boilers, and other pressure vessels associated piping systems and fittings shall be of a design and construction adequate for the service for which they intended and shall be installed and protected as to reduce to a minimum any danger to persons on board. The PSCO observed leaking packing glands on the general service fire pump and #3 salt water cooling pump. The salt water discharge on the main engine oil cooler was observed to have a pin hole leak and water was observed leaking from the salt water drain pipe. 74 SOLAS (14) II-1/26.1 Code: 17 a & c

The vessel shall have an installed fixed fire detection system that is in serviceable condition. 05 dectors were identified as damaged, cracked, or missing the protective covers SOLAS 74 (14consol) II-2 / 7.2.3

The vessel shall limit the fire growth potential in every space on the ship. Oil soaked lagging on transfer pumps, fuel oil transfer systems was excessive. Lagging shall be renewed as appropriate. SOLAS 74 (14 consol) II-1 / 5

The machiner, boilers, other pressure vessels and associated piping systems and fittings shall be of design and construction adequate for the service for which intended and shall be so installed and protected as to reduce to a minimumto any person on board. The boiler has 2 significant steam leaks at the flange connection. SOLAS 74 (14 consol) II-1 / 26.1

Electrical apparatus shall be so constructed and so installed as not to cause injury. Plastic bag was placed over the engine room alarm to shield the fixture from a leaking steam line. SOLAS 74 (14 consol) II-1 / 45.1.3

Piping systems and fittings shall be of design and constrcution adequate for service. Seawater pumps and associated piping/flanges leaking excessively. SOLAS 74 (14 consol) II-1 / 26.1
