When PSCO requested to test fixed gas detection, crew applied 60% LEL methane span gas. System read 107%, 32%, and 54% on the Entrance to accommodation space D deck, GVU room, and the boiler fuel double pipe 1 fixed gas sampling points, respectively, of 04 sampling points tested on GS3000 panel.

Gas detection equipment should be so designed that it may readily be tested. Testing and calibration should be carried out at regular intervals. Suitable equipment and span gas for this purpose should be carried on board, When PSCO requested to test fixed gas detection in the SW2020 panel, crew applied 60% LEL methane span gas. System read 72% LEL. When PSCO witnessed calibration on the #1 sensor, the calibration failed. Procedures stated that manual calibration must be completed if calibration fails. The ship does not have equipment to calibrate onboard. This is a redundant system and #3 LEL sensor is operational.

The safety management system should ensure compliance with mandatory rules and regulations. The inspections mentioned in 10.2 as well as the measures mentioned in 10.3 should be integrated into the ship's operational maintenance routine. Ship's ISM procedure for the gas detection calibration states "a calibration check should be performed on the fixed gas detection equipment monthly, or before arrival in port, whatever is first." The vessel has been testing the fixed gas detection by zones, only 1 zone per month and the entire system every 6 months. In addition, PSCO observed three of four gas detection units randomly selected were not reading properly. Recommend an internal audit of vessel's safety management plan.
