No electrical equipment shall be installed in any space where flammable mixtures are liable to collect including those on board tankers... unless the administration is satisfied that such equipment is appropriate to the space concerned. 6 explosion proof light lenses were found cracked in the cargo pump room. 1 light above the #1 COP separator, 1 light above the ballast water pumps, 2 lights above the stairs on the 01 deck above the pump room bottom, 1 light on the 02 deck by the cargo valves, and 1 light on the 04 deck near the cow heater. The vessel is not authorized to discharge cargo in the United States until rectified. 74 SOLAS (04) II-1/45.10.3

Resolution: Class attended the vessel and verified that all 6 lights were electrically isolated from the panels, and spares were on order.

Every company should develop, implement and maintain a safety management system which includes procedures for reporting non-conformities with the provisions of this code. The vessel properly reported a malfunctioning R.O.T tank radar to the company, but could not produce proof that the class or flag was ever notified of the condition, nor mitigating action was taken. An external audit should be conducted to verify appropriate reporting of non-conformities. 74 SOLAS (20) IX/3.1 - ISM Code 1.4.3

Resolution: Electrician repaired the junction box and sealed the wires in so they couldn't be pulled back out.
