Where penetrations of watertight bulkheads and internal decks are necessary for access etc., arrangements are to be made to maintain watertight integrity. The ship's five watertight doors below the bulkhead deck require gasket renewal and/or door adjustment to make the doors watertight. Several of the gaskets are torn and damaged, making the doors non-watertight. The deficiency was originally identified by the ship to the company in November 2018 and repairs have not been completed.

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. During the lowering, release, and operation of the rescue boat, the following problems were observed: The designated rescue boat coxswain had not lowered and operated the boat prior to the inspection; the rescue boat crew attempted to release the boat prior to the boat being fully in the water without success. The Chief Mate had to coach the rescue boat crew to fully lower the boat. When the boat was fully lowered the crew could still not release the boat because the boat was lowered too slowly, preventing enough slack in the fall to allow for release. On the third lowering, the boat was released. The coxswain then came full ahead on the boat while the painter and a frapping line were still attached, creating a dangerous situation and pulled the lines into the water.
