Resolved 4 May 2019
Resolution: Inert gas system was re-tested by PSCO and found to be producing less than 5% oxygen at the source.

Every crew member with assigned emergency duties shall be familiar with emergency training and drills. Crew attempted and failed two fire drills. Crew cannot demonstrate to PSCO during fire drills proper fire fighting procedures and techniques. Crew is also not familiar with properly donning immersion suits. SOLAS74(2014 CONS) Ch. III/19.2.1 17c

Resolution: Crew was able to demonstrate satisfactory fire drill and donning immersion suits upon returning back to vessel the following day.
