Electrical installations shall be that the safety of passengers, crew, and ship form electrical hazards will be ensured. Breaker lockout devices were installed in an upside down position on the No. 2 freshwater cooling pump and No. 4 engine room supply fan systems, both of which were energized. This would have prevented the tripping of the system in the event of a short circuit.
