Each incinerator on a ship constructed on or after 01 January 2000 shall meet the requirements contained in appendix IV to this Annex. Each incinerator subject to this paragraph shall be approved by the Administration taking into account the standards developed by the organization. PSCO observed that the refractory brick in the incinerator was significantly deteriorated. MARPOL VI/16.6.1

The protection of the cargo tanks shall be achieved by a fixed inert gas system IAW the requirements of the Fire Safety Systems Code. The inert gas system shall be designed, constructed, and tested to the satisfaction of the administration. It shall be designed to be capable of rendering and maintaining the atmosphere of the relevant cargo tanks non-flammable. PSCO observed IGS not functioning properly IAW design specifications, to include inoperable blowers and valves in automatic mode. 74 SOLAS (20) 11-2/4.5.5.1.1 FSS CODE 15.2.2.1.1

Resolved 2 July 2024
Resolution: PSCO observed class survey report attesting to repairs made to the IGS. PSCO conducted follow-up exam on vessel and witnessed satisfactory operation and testing of IGS. All sat.

Every steam pipe and every fitting connected thereto through which steam may pass shall be designed, constructed and installed as to withstand the maximum working stresses to which it may be subjected to. PSCO observed excessive steam leak in engine room in vicinity of engine control room. 74 SOLAS (04) 11-1/33.1

Issued 19 April 2023 Resolved

The inert gas system shall be designed, constructed and tested in accordance with the fire safety sytems code. Special consideration shall be given to the design and location of scrubber and blowers with relevant pipping and fittings in order to prevent flue gas leakages into enclosed spaces. PSCO observed water and flue gas leakage at crack at the inlet into the scrubber chamber.

Action required: 60 - Rectify deficiencies prior to movement
Resolved 19 April 2023
Resolution: Vessel conducted temporary weld repairs, operational tests were satisfactory. Oustanding Condition of Class has been created, permanant repairs to be completed within 30 days. Code 60 has been cleared.
