Bulk carriers shall comply with the maintenance requirements provided in regulation II-1/3-1 and the standards for owners inspection and maintenance of bulk carrier hatch covers, adopted by the organization by resolution MSC.169(79). Psco observed hatch ventilation covers unable to be opened by crew. 74Solas(20)XII/7.2

A SECOND SOURCE OF ENERGY SHALL BE PROVIDED FOR AN ADDITIONAL THREE STARTS WITHIN 30 MINUTES OR LESS. PSCO OBSERVED SECOND MEANS OF START UNABLE TO OPERATE. 74 SOLAS (04) CHPT II-1/REG 44.2

GUARD RAILS FITTED ON SUPERSTRUCTURE AND FREEBOARD DECKS SHALL HAVE AT LEAST THREE COURSES. PSCO OBSERVED MISSING CHAIN COURSES ON PORT SIDE GUARD RAIL. ICLL 25.3

ALL HATCHWAYS EXCEPT AS PROVIDED IN REGULATION 14(2), SUCH COVERS SHALL BE WEATHERTIGHT AND FITTED WITH GASKETS AND CLAMPING DEVICES. THE ARRANGEMENTS SHALL ENSURE THAT THE TIGHTNESS CAN BE MAINTAINED IN ANY SEA CONDITIONS. PSCO OBSERVED WASTAGE OF HATCH COVER CLEAT BRACKETS BETWEEN HATCH COVERS 1 AND 2 AND 2 AND 3. ICLL 16.1

SYSTEMS AND EQUIPMENT, INSTALLED ON OR AFTER 1JULY2002 TO PREFORM THE FUNCTIONAL REQUIREMENT OF REGULATION 20 SHALL CONFORM TO APPROPRIATE PERFORMANCE STANDARDS NOT INFERIOR TO THOSE ADOPTED BY THE ADMINISTRATION. PSCO OBSERVED THAT ECHO DEPTH SOUNDER AND AND AUTOPILOT WERE NOT ALARMING ON VDR. 74SOLAS(20)V/18.2

SHIPS SHALL BE DESIGNED, CONSTRUCTED, AND MAINTAINED IN COMPLIANCE WITH THE STRUCTURAL MECHANICAL, AND ELECTRICAL REQUIREMENT OF A CLASSIFICATION SOCIETY WHICH IS RECOGNIZED BY THE ADMINISTRATION. PSCO OBSERVED THE SHIPS RUDDER ANGLE INDICATORS IN VARIOUS LOCATIONS ONLY DISPLAYED 35 DEGREES STARBOARD TO 30 DEGREES PORT. 74SOLAS(09)II-1/3-1

Garbage collected throughout the ship should be delivered to designated processing or storage locations. PSCO observed garbage collection in bow thruster room and stowage of garbage in thruster room is not indicated as approved for stowage in Garbage Management Plan

Fire protection systems and fire fighting systems and appliances shall be maintained ready for use. PSCO was unable to operate emergency fire pump due to excessive accumulation of fire fighting water in space.

The Company should establish procedures to ensure that the ship is maintained in conformity with the provision of the relevant rules and regulations and with any additional requirements which may be established by the Company. Objective evidence revealed that documented procedures and instructions do not take into account international conventions, Flag and Port State regulations, classification rules, requirements from manufacturers, and feedback information from failures, damages, defects and malfunctions.

The Company should establish procedures for the implementation of corrective action. Objective evidence revealed that records of non-conformities, accidents and hazardous situations are not being reported in accordance with established SMS procedures. Evidence of documented corrective actions including actions to prevent recurrence are not being carried out in accordance with SMS procedures. PSCO noted a lack of effectiveness and timely implementation of corrective action for previous Flag State and Port State identified non-conformities and deficiencies.

In meeting the requirement of ISM Code 10.1, the Company should ensure that any nonconformity is reported, with its possible cause, appropriate action is taken, and records of these activities are maintained. Records of inspections, maintenance, damages, defects, and relevant corrective actions were not available for review as objective evidence of the effective functioning of the SMS. Flag State, Port State, and classification society are not being notified of vessel defects, as defined in SMS Chapter 12, for damage or faulty operation of equipment which could endanger or has compromised the safety of people, the environment, and the vessel or its property.

A main source of electrical power of sufficient capacity to supply all those services mentioned in regulation 40.1.1 shall be provided. This main source of electrical power shall consist of at least two generating sets. PSCO observed an unreported hazardous condition aboard subject vessel in that the number one auxiliary generator is inoperable since October 19th due to a catastrophic failure of connecting rod and the cylinder block casing in the area of cylinder no. 5. 74SOLAS(04)II-1/41.1.1

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present Convention to ensure that the ship in all respects will remain fit to proceed to sea without presenting an unreasonable threat of harm to the marine environment. PSCO observed that the dates on the Oil Content Meter are in contrast to recorded entries in the Oil Record Book part 1. MARPOL I/4.1

Ships of 500 gross tonnage and upwards shall be provided with at least one international shore connection complying with the Fire Safety Systems Code. PSCO observed that the international shore connection did not have the required washers.74SOLAS(20)ii-2/2.1.7.1 FSS Code 2.2

Whenever there is a hazardous condition either on board a vessel or caused by a vessel or its operation, the owner, agent, master, operator, or person in charge must immediately notify the nearest Coast Guard Sector Office or Group Office, and in addition submit any report required by 46CFR.05-10. Vessel failed to report a catastrophic failure of the number 1 auxiliary generator. 33CFR160.216(a)

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCOs observed that the rescue boat engine was inoperable. 74 SOLAS (2020) III/20.2

...A second source of energy shall be provided for an additional three starts within 30 minutes unless manual starting can be demonstrated to be effective. PSCOs observed that the crew was unable to start the emergency generator using the secondary means.

An alarm system shall be provided indicating any fault requiring attention and shall be capable of sounding an audible alarm in the the main machinery control room or at the propulsion machinery control position. PSCO discovered 02 bilge alarms inoperable.

The condition of the ship and it;s equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO discovered 01 CO2 bottle on the 4th rack without safety cap.

Ships shall be designed, constructed and maintained in compliance with the structural, mechanical and electrical requirements of a classification society which is recognized by the Administration. Echo depth sounder will not maintain a constant display. Display is intermittent will not hold depth on screen.

The arrangements provided shall be such as to ensure that the safety of the ship in all sailing conditions, including maneuvering is equivalent to that of a ship having the machinery spaces manned. PSCO noted bags of trash and paint cans stored behind switch board.

Before the ship leaves and at all times during the voyage, all life saving appliances shall be in working order and ready for immediate use. PSCO discovered embarkation lights inoperable.

01102 - Cargo Ship Safety Construction (including exemptions) The condition of the ship and its environment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed 5 degree port side deviation from rudder angle indicator to rudder post indicator in steering gear room. 74 SOLAS (09) I/11a

All ships of 300 gross tonnage and upwards and passenger ships irrespective of size shall, in addition to meeting the requirements of paragraph 2.2, be fitted with: an echo sounding device, or other electronic means, to measure and display the available depth of water. Vessel's echo depth sounder is inoperable to due to defective transducer, to be repaired at next Drydock.
