PSCO OBSERVED PORT AND STARBOARD FORWARD LIFE RAFT EMBARKATION LIGHTS WITH EXPOSED WIRING IN THE HAZARDOUS AREA.

Instrumentation shall be fitted for continuously indicating and permanently recording when inert gas is supplied: the oxygen content of the inert gas. The vessel was unable to complete a calibration of the oxygen analyzer and demonstrate the system was able to deliver an inert gas with an oxygen content of not more than 5%. Each cargo tank of a tankship must be equipped with high level and overfill alarms that have audible and visual alarm indications that can be seen and heard on the vessel where cargo is controlled. Indicators for cargo tanks 1P and 2S were malfunctioning and constantly alarming. The cite used for this deficiency was 46 CFR 39.20-7.
