All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner (74 SOLAS (2004) II-1/45.1.3) PSCO Observed sparking from an exposed wire on an external light fixture just before the life boat embarkation deck. Exposed wire made constact with bracket causing small fire.
Condition: Improper/Lack of Maintenance

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Rescue boat could not be recovered by davit motor. Crew had to manually hoist rescue boat. Additionally, winch brake could not support load of rescue boat and had to be held by a crew member. 74SOLAS(14 CONS) Ch. III/Reg.20.2 17ac

Maintenance, testing and inspections shall be carried out based on the guidelines. The crew was unable to demonstrate proper operation of the flame detector in the purifer space, could not find the corresponding manual for the system. SOLAS 74 (04) II-2/ 14.2.2.1

Resolution: Received report from class. Vessel has obtained operating manual for Flame Detector. Class tested Flame Detector and water mist system. All satisfactory.
