The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: fire detection and fire alarm systems. PSCO observed a malfunctioning smoke detector via the fire panel on the bridge. The fault did not clear when the detector was changed. All other detectors were functioning properly. Faulty detector located in crew laundry. SOLAS (09) II-2/14.2.1.1.2 Action: 16ac

In meeting these requirements, the company should ensure that records of these activities are maintained. PSCO observed objective evidence that despite maintenance being conducted, logging such maintenance into the ship's Preventative Maintenance System is not occuring in accordance with the SMS. Ship's crew also admitted they often conduct maintenance without properly logging it. This deficiency must be addressed in additional audit prescribed by BV Report No: MIA0/ENO/20251225211057, not to exceed by date 26MAR2026. SOLAS (20) IX/3 ISM Code Part A 10.2.4

The company should ensure that all personnel involved in the company's safety management system have an adequate understanding of the relevant rules, regulations, codes and guidelines. The vessel did not notify the Company of the sheared steering gear and system failure in accordance with the established procedures of the Safety Management System (SMS). There is no objective evidence to indicate that the damaged/temporarily repaired steering system was properly reported to the Recognized Organization (RO) or Administration to ensure appropriate corrective actions were taken in accordance with the ship's SMS. Further review indicated 169 overdue PMS items with 17 overdue critical jobs. A safety management audit carried out by the Administration or the RO, with corresponding report of findings satisfactory to the OCMI must be submitted prior to clearance of this deficiency and release from detention.

All steering gear components and the rudder stock shall be of sound and reliable construction to the satisfaction of the Administration. During the exam, it was noted that the steering ram had completely sheered and been temporarily welded by the crew, without a properly obtained hotwork permit per the SMS. The ship is operating on a full term safety construction certificate and there was no evidence to indicate that this repair had been accepted by RO/flag. It should be noted that Bureau Veritas class/statutory renewal surveys were completed on 14DEC2025.

To prevent the probability of ignition of flammable or combustible liquids, means shall be provided to control leaks and ignition sources shall be restricted. PSCOs observed a slow flow, steady leak from the oil purifier room booster unit.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCOs observed the following gauges inoperable: (1) booster unit pump (2) suction side lube oil separator pump (3) heavy fuel oil transfer pumps (4 gauges in total).

The purpose of this regulation is to contain a fire in the space of origine. The fire integrity of the divisions shall be maintain at openings and penetrations. PSCOs observed an open cable run in the overhead of the ECR behind the main switchboard. 74SOLAS(20) II-2/9.1.3 10c

0615: Before the ship leaves and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed tha primer bulb for the rescue boat's fuel syatem was cracked. Damage to bulb allowed air into the line not allowing the engine to properly start. SOLAS 78(14) CHIII 20.2

0799 - Fire Fighting - Doors required to be self-closing shall not be fitted with hold-back hooks. Found multiple doors with hold-back blocks in place. 74SOLAS(2009) CH II-2/4.2.2

Vessel does not have bunker transfer procedures as per 33 CFR 155.750. Prior to conducting bunkering operations provide procudures to attending USCG inspector
