PSCO observed a malfunctioning smoke detector via the fire panel on the bridge. The fault did not clear when the detector was changed. All other detectors were functioning properly. Faulty detector located in crew laundry.

PSCO observed objective evidence that despite maintenance being conducted, logging such maintenance into the ship's Preventative Maintenance System is not occuring in accordance with the SMS. Ship's crew also admitted they often conduct maintenance without properly logging it.
This deficiency must be addressed in additional audit prescribed by BV Report No: MIA0/ENO/20251225211057, not to exceed by date 26MAR2026.

The company should ensure that all personnel involved in the company's safety management system have an adequate understanding of the relevant rules, regulations, codes and guidelines. The vessel did not notify the Company of the sheared steering gear and system failure in accordance with the established procedures of the Safety Management System (SMS). There is no objective evidence to indicate that the damaged/temporarily repaired steering system was properly reported to the Recognized Organization (RO) or Administration to ensure appropriate corrective actions were taken in accordance with the ship's SMS. Further review indicated 169 overdue PMS items with 17 overdue critical jobs.
A safety management audit carried out by the Administration or the RO, with corresponding report of findings satisfactory to the OCMI must be submitted prior to clearance of this deficiency and release from detention.

All steering gear components and the rudder stock shall be of sound and reliable construction to the satisfaction of the Administration. During the exam, it was noted that the steering ram had completely sheered and been temporarily welded by the crew, without a properly obtained hotwork permit per the SMS. The ship is operating on a full term safety construction certificate and there was no evidence to indicate that this repair had been accepted by RO/flag. It should be noted that Bureau Veritas class/statutory renewal surveys were completed on 14DEC2025.
