THE COMPANY SHOULD ESTABLISH PROCEDURES TO ENSURE THAT THE SHIP IS MAINTAINED IN CONFORMITY WITH THE PROVISIONS OF THE RELEVANT RULES AND REGULATIONS AND WITH ANY ADDITIONAL REQUIREMENTS WHICH MAY BE ESTABLISHED BY THE COMPANY. THE TECHNICAL AND OPERATIONAL NATURE OF DEFICIENCIES TWO AND THREE PROVIDE OBJECTIVE EVIDENCE OF A SERIOUS FAILURE OF IMPLEMENTATION OF THE ISM CODE. AN EXTERNAL AUDIT REPORT SATISFACTORY TO THE OCMI MUST BE SUBMITTED PRIOR TO CLEARANCE OF THIS DEFICIENCY.

FIREFIGHTING SYSTEMS AND APPLIANCES SHALL BE KEPT IN GOOD WORKING ORDER AND READILY AVAILABLE FOR IMMEDIATE USE. PSCO OBSERVED FIRE DETECTION ON BRIDGE WAS BEING UPGRADED AND DURING CARGO OPERATIONS. INITIAL REPORT TO COMPANY WAS MADE IN SEPTEMBER, 2025. PROPER PROCEDURES WERE NOT FOLLOWED. FIRE DETECTION SYSTEM WAS INOPERABLE AFTER SEP2025 DUE TO OVERHEATING.

ADDITIONALLY LOOSE/ IMPROPERLY INSTALLED WIRING FOR MOTOR ROOM VENTILATION FANS.

Resolution: Recieved class report attesting to repairs. Attended vessel and identified 06 electrical installations that had not been repaired. Crew repaired to the satisfaction of PSCO and attending class surveyor
