FIREFIGHTING SYSTEMS AND APPLIANCES SHALL BE KEPT IN GOOD WORKING ORDER AND READILY AVAILABLE FOR IMMEDIATE USE. PSCO OBSERVED FIRE DETECTION ON BRIDGE WAS BEING UPGRADED AND DURING CARGO OPERATIONS. INITIAL REPORT TO COMPANY WAS MADE IN SEPTEMBER, 2025. PROPER PROCEDURES WERE NOT FOLLOWED. FIRE DETECTION SYSTEM WAS INOPERABLE AFTER SEP2025 DUE TO OVERHEATING.

ADDITIONALLY LOOSE/ IMPROPERLY INSTALLED WIRING FOR MOTOR ROOM VENTILATION FANS.
