A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel is overdue for drydock examination date. Vessel is prohibited from carrying passengers until drydock examination is complete. 46 CFR 176.600(c)(1)

The OCMI may require the vessel to get underway. Unable to conduct "man overboard" and fire drill due to issue with throttle. Conduct drills to the satisfaction of the attending Marine Inspector. 46 CFR 176.404(a)

The owner or managing operator must provide satisfactory evidence of the required servicing to the Marine Inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility must perform the required inspections, maintenance procedures, and hydrostatic pressure tests. Marine inspector observed 02 portable fire extinguishers with expired servicing tags. Provide evidence that portable fire extinguishers have undergone required servicing in accordance with §176.810(b), to the satisfaction of the attending Marine Inspector. 46 CFR 176.810(b)(1)(iv)

Provisions must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. Marine Inspector observed insufficient means for ventilation closures for engine compartment. (1) Provide written proposal for ventilation closures, (2) Once approved, install, to the satisfaction of the attending Marine Inspector.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: (2) A machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space. Marine Inspector observed 01 bilge alarm did not have an operable visual indicator at operating station. Repair/Replace visual indicator, to the satisfaction of the attending Marine Inspector.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Marine Inspector observed the masthead light was inoperable. Repair/replace light, to the satisfaction of the attending Marine Inspector.

Firefighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Vessel was unable to provide documented evidence of fire fighting drills or training. Provide drill and training logs, to the satisfaction of the attending Marine Inspector. 46 CFR 185.524(d)

A vessel to which this subchapter applies may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection. Vessel is overdue for COI Annual Inspection, invalidating the Certificate of Inspection.

A number of child size life jackets equal to at least 10% of the number of persons permitted on board must be provided. The child life jackets were unsuitable for use. Replace child life jackets to the satisfaction of the attending Marine Inspector. 46CFR180.71(b)

A vessel must comply with the applicable requirements for any radio installations, including the requirements for installation certificates. 46CFR184.502

Abandon ship and man overboard drills and training shall be logged or otherwise documented. The drill entry shall include the following information:(1) Date of the drill and training; and(2) General description of the drill scenario and topics. Drills are not documented. Provide of drills to the satisfaction of the attending Marine Inspector. 46CFR185.520

A space containing propulsion machinery must be equipped with a fixed gas fire extinguishing system in compliance. The engine space fire boy is not operational. Make repairs prior to the carriage of passengers. 46 CFR 181.400

Portable fire extinguishers must be inspected and maintained. The portable fire extinguishers have not been serviced. Service extinguishers prior to the carriage of passengers. 46 CFR 176.810

An adult life jacket must be provided for each person carried on board a vessel. In addition, a number of child size life jackets equal to 10% of the number of persons permitted must be provided. One of the adult and one the child life jackets were wasted and need to be replaced. Adult passenger count reduced to 16 and passengers under 90 pounds reduced to 01. 46 CFR 180.71(a), (b)

The calculations used to determine stability information, must be submitted in addition to the applicable information listed in 46CFR178.210(b). The vessel changed engines and there is an associated weight difference. Complete a simplified stability proof test. 46CFR178.210(b)

When the flexible section of a fuel fill pipe is a non-conductor of electricity, the metallic sections of the fill pipe separated thereby shall be joined by a conductor. Both fuel tank fill pipes are separated by flexible hose without being joined by a conductor. Install proper conductors. 46CFR182.445(g)

The name of the vessel must be marked on the port and starboard bow, and the stern of the vessel. The hailing port of the vessel must be marked on the stern of the vessel. Name and hailing port not marked on the vessel. Place name and hailing port in correct locations. 46CFR185.602(a) 46CFR67.123(a)

At each inspection the the owner or manager must have the vessel ready for inspection. The vessel was still in the yard being worked on and the inspection could not be completed. Make the vessel ready for inspection. 46CFR176.804

At each inspection of a vessel, the owner or managing operator shall be prepared to conduct tests including the following: Inspection and operation of through hull fittings and sea valves. Sea valves unable to close completely. Replace all sea valves. 46 CFR 176.802(a)(2)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] aft swim platform removed from vessel, reinstall structure. 46 CFR 176.700(a)

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Dead end wires found in engine and fwd compartment. Remove dead end wiring. 46 CFR 183.200 (c)

Each EAP education program must include at least the following elements: display and distribution of a community service hot-line telephone number for crew member assistance. Vessel does not have a community service hot-line telephone number for crew member assistance posted onboard. Post hotline number was not on board the vessel. 46 CFR 16.401(a)

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Missing all three required buckets. Provide fire buckets. 46 CFR 181.610

All vessels must have operable navigation lights in accordance with the Navigation Rules. The vessel’s side navigation lights were extinguished. Fix lights. 46 CFR 183.420

The owner or operator must conduct all tests as required by the OCMI, and make the vessel available for all specific inspections and drills required by subpart H of this part. In addition, the OCMI may require the vessel to get underway. Get underway and conduct drill to the satisfaction of the attending MI. 46 CFR 176.404(a)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Per ASTM F3099-19 4.1, vessels used for parasailing operations shall be equipped with a weather monitoring device. Vessel did not have a weather monitoring device onboard. Affix a weather monitoring device onboard. 46 CFR 176.840

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Per ASTM F3099-19 5.2.3, vessels used for parasail operations shall have a secondary anchor or equivalent device to augment the primary anchor onboard. Vessel did not have a secondary anchor onboard. Install secondary anchor. 46 CFR 176.840

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Per ASTM F3099-19 5.3.3.1, parasail harness, passenger bars, and canopies shall be sent to the manufacturer every year to be overhauled and inspected. Except for 2 of the 19 passenger harnesses, there was no evidence of manufacturer’s overhaul and inspection of the above listed equipment. Provide evidence of the manufacturer’s overhaul and inspection for the parasail harness, passenger bars, and canopies. 46 CFR 176.840

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Per ASTM F3099-19 7.1.2, A minimum of one crewmember onboard shall be certified and current in an accepted course of standard First Aid including Adult-Child-Infant cardiopulmonary resuscitation (CPR). Crew member was missing CPR certification. Crewmember must provide current CPR certification. 46 CFR 176.840

A Certificate of Documentation (COD) is required for the operation of a vessel in certain trades, serves a sevidence of vessel nationality, and permits a vessel to be subject to preferred mortgages. Vessel Operator could not produce original COD. Provide the original COD. 46 CFR 67.1

Portable watertight containers for distress flares and smoke signals shall be of a bright color, and containers shall be clearly marked in legible contrasting letters at least 12.7 millimeters (0.5 inches) high: “DISTRESS SIGNALS”. Portable watertight container for distress flare labeling faded. Renew label. 46 CFR 185.614

Each vessel must be equipment with at least two operable portable battery lights. Vessel did not have a second portable battery light. Provide a second portable battery light. 46 CFR 183.430

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. The vessel’s first-aid kit is lacking ammonia inhalants and iodine applicators. Renew first-aid kit supplies onboard the vessel. 46 CFR 184.710

Each ship transporting more than six passengers for hire equipped with a radiotelephone station must carry a radio operator who: holds a marine radio operator permit or higher class license. The master onboard did not have proof of valid radio operator permit. Provide proof of valid radio operator permit. 47 CFR 80.158 (e)

A suitable metal marine type strainer, meeting the requirements of the engine manufacturer, must be fitted in the fuel supply line in the engine compartment. Strainers must be leak free. Strainers must be the type of opening on top for cleaning screens. A drip pan fitted with flame screen must be installed under gasoline strainers. Fuel filter and strainer bowls must be highly resistant to shattering due to mechanical impact and resistant to failure due to thermal shock. Fuel filters fitted with bowls of other than steel construction must be approved by the Commandant and be protected from mechanical damage. Approval of bowls of other than steel construction will specify if a flame shield is required. Fuel filter does not have a strainer bowl. Install an approved strainer bowls. 46 CFR 182.455(b)(6)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Loss of propulsion and re-inflation of parasail with customers inside on 15 October 2021 has caused safety culture to be called into question. Owner must schedule USCG attendance and demonstrate adherence to ASTM F3099 to attending Marine Inspectors. 46 CFR 176.840

Nothing in this subpart limits the marine inspector from conducting such tests or inspections he or she deems necessary to be assured of the vessel's seaworthiness. Vessel experienced a loss of propulsions, causing the crew to water-land the rig with passengers inside. Vessel is restricted from operating until deemed safe by Marine Inspector, to include witnessing proper operation of vessel's propulsion and fuel systems. 46 CFR 176.500(b)(iv)

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. The vessel's Annual Inspection date was past due as of 12 March 2021. Accordingly, all privileges afforded by the vessel's COI are discontinued and the vessel is restricted to the carriage of not more than six passengers, including at least one for hire. A COI Inspection must be completed prior to re-issuance of the vessel's Certificate of Inspection. 46CFR176.500(b)(1)

A vessel must ensure that the material condition is satisfactory for its intended service. Minor crack in fiberglass observed in vicinity of bow thruster tunnel. Provide repair proposal to satisfaction of attending Marine Inspector.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Main engine would not start. Vessel is restricted from commercial operation until the main engine is repaired, the fire boy testing is completed, and completion of sea trials.

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. The bilge alarm fuse panel must be replaced with a panel that uses cartridge type fuses.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. The VHF radio does not receive a GPS position.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. A maintenance log of the monthly portable fire extinguishers checks is required.

Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. A monthly testing log for the EPIRB is required.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. The aspirin inside the first-aid kit had expired.

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Vessel's ring life buoy is missing the vessel's name.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Engine coolant is leaking into bilge and large oil droplets are covering port side of the engine. Identify and repair both engine coolant leak and oil leak.

BIG O experienced an oil leak from an unknown location into the engine compartment while underway, resulting in unusual sounds and degraded performance of the engine. Prove proper operation of propulsion machinery prior to the carriage of passengers.

DURING DRYDOCK EXAM, CAVITATION WAS NOTICED ON PROPELLER BETWEEN PROPELLER HUB AND BLADES. REPLACE PROPELLER DUE TO CAVITATION.

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating; vessel's aft automatic bilge pump indicator light is not working. Prove proper operation of visual indicator.

Placards containing instructions for the donning and use of life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers; Provide instruction placard for the donning of life jackets.

Each life jacket , immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name; Write the name on vessel's ring life buoy.

Provide service report for pre-engineered fire extinguishing system in engine room.

Provide proper opperation of Fireboy audible/visual alarm.

Correct call sign on safety radio certificate.

Provide three fire buckets.

Provide nautical charts and tides tables for operational area.

Demonstrate proper operation of vessel propulsion: main and alternate controls.
