Each OSV must carry a category 1 406 MHz satellite EPIRB meeting the requirements of 47 CFR part 80. Vessel was not equipped with an EPIRB. Owner rep to install in float free location and plainly mark with vessel name.

The master shall conduct sufficient drills and give sufficient instruction to make sure that all crewmember are familiar with their duties during emergencies that necessitate abandon ship or the recovery of persons who have fallen overboard. Abandon ship drills and training shall be logged or otherwise documented. The drill entry shall include the date of the drill and training and the general description of the drill scenario and training topics. The vessels master could not provide logs or evidence of man overboard or abandon ship drills by required time lines. Vessel shall conduct drills and provide records prior to getting underway to the satisfaction of the Coast Guard. 46 CFR 185.520(a) & (f)

The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties. Fire Fighting drills and training shall be logged or otherwise documented. The drill entry shall include the date of the drill and training and the general description of the drill scenario and training topics. The vessels master could not provide logs or evidence of firefighting drills on required timelines. Vessel shall conduct drills prior to getting underway and provide records to the satisfaction of the Coast Guard. 46 CFR 185.524(a) & (d)

The owner or master shall instruct each crewmember, upon first being employed for the first time on a particular vessel and at least once every three months as to the duties the crewmember is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard. Crew training shall be logged or otherwise documented. The training entry shall include the date of the training and the general description of the training topics. The vessels owner or master could not provide logs or evidence of training by the required timelines. Vessel shall conduct training prior to getting underway to the satisfaction of the Coast Guard. 46 CFR 185.420(a) & (c)

Each "L" vessel will be equipped with one (1) fire axe located in or adjacent to the pilothouse. Vessels fire ax handle was discovered wasted. When tested, the handle broke at the should of the handle. Vessel shall replace fire ax. 46 CFR 181.35-1

One length of fire hose shall be provided for each fire hydrant required. During pressure test, fire hose ruptured at nozzle end. Corrected on the spot. 46 CFR181.15-10(a) Old subchapter T.

Vent pipes for diesel fuel tanks shall be fitted with a corrosion resistant 30 x 30 mesh. Screen on port fuel tank was worn exposing a large opening in center of mesh wire. Corrected on spot. 46 CFR 182.20-35(d) Old subchapter T

Portable fire extinguishers must be installed to the satisfaction of the marine inspector. Engine room CO2 extinguisher mounting bracket was missing securing pin preventing extinguisher from falling out. Corrected on the spot. 46 CFR 181.520

Every vessel must be fitted with a suitable magnetic compass. Compass mounted at operating station had a severely faded lens making it difficult to obtain heading. 46 CFR 184.402

Vessel must carry up to date charts appropriate for the intended voyage. A chart for the operating area was provided, but needed updated.

Vessel must carry adequate and up to date notice to mariners. Vessel did not have publication on board.46 CFR 185.20-35 Old subchapter T.
