The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. Vessel had ‘LEAKAGE AT ME FUEL PIPE’ alarm blocked. Crew stated main engine injectors and fuel pumps were serviced by OEM after yard period, resulting in a fuel, source unknown (Class NK report #26EJ0214 dated March 13, 2026). MI ran main engine and noticed fuel oil leaking steadily from leak oil piping drain into casing catch and no signs of external fuel oil leaks from exposed components. Fuel oil leakage alarm read ‘Normal’ and operated satisfactory at alarm condition. Investigate fuel oil leak from internal casing and conduct repairs with servicing OEM.

In tankers, electrical equipment, cables and wiring shall not be installed in hazardous locations unless it conforms with standards not inferior to those acceptable to the Organization. Vessel’s three (03) explosion proof lights on the forward bulkhead had salt water sediment in the containment indicating water intrusion. Replace three (03) lights with explosion proof rated lighting.

Cylinders must always be secured and, when not in use, they must be stowed in a rack in an upright position. Vessels oxygen cylinders were stowed directly on the deck and secured via a ratchet strap not in a designated rack. Move DOT cylinder into a storage bracket.

The location of the controls of any valve serving a sea inlet, a discharge below the waterline or a bilge injection system shall be so sited as to allow adequate time for operation in case of influx of water to the space, having regard to the time likely to be required in order to reach and operate such controls. If the level to which the space could become flooded with the ship in the fully l Remote access valve for bow thruster space emergency bilge educator was seized open and the connection point "knuckle" is sheared at the valve stem not permitting remote activation. Replace valve and retest bilge educator operating for bow thruster space.

Electrical installations shall be such that: electrical services essential for safety will be ensured under various emergency conditions. CO2 room ventilation local controller is inoperable.

Posters or signs shall be provided on or in the vicinity of survival craft and their launching controls. During vessel’s pressure washing and exterior house maintenance, required placards postage were damaged. The master or person in charge of a ship, including a drilling rig or platform, to which this section applies shall ensure that one or more placards meeting the requirements of this section are displayed in prominent locations and in sufficient numbers so that they can be read by the crew and passengers. Replace life saving instructions for life jacket donning and survival craft operating instructions and garbage discharge placard at appropriate locations.

General arrangement plans shall be permanently exhibited for the guidance of the ship's officers, showing clearly for each deck the control stations, the various fire sections enclosed by "A" class divisions, the sections enclosed by "B" class divisions together with particulars of the fire detection and fire alarm systems, the sprinkler installation, the fire-extinguishing appliances, means of access to different compartments, decks, etc., and the ventilating system, including particulars of the fan control positions, the position of dampers and identification numbers of the ventilating fans serving each section. Fire control plan reflects deck water spray system which has been decommissioned and removed from onboard fixed fire fighting systems. Vessel utilized washroom space as a storage space for consumables. Conduct Fire Control Plan review of removed deck water spray system and space categorization for use of storage space, instead of washroom, to validate structural fire protection rating.

Test procedure documents must be in a step-by-step or checkoff list format. Each test instruction must specify equipment status, apparatus necessary to perform the tests, safety precautions, safety control and alarm setpoints, the procedure to be followed, and the expected test result. Several areas on the PSTPs did not reflect onboard testing conditions and approved testing procedures including: steering power failure alarm stated 'standby pump starts' and did not start (2-C-5), no EOT power failure alarm included in test procedure (2-A), ESTOP for boiler did not include bridge location (7-A-7), ESTOP for boiler states TMP-2 action (7-A-7), fuel oil pressure alarm for boiler states PRS-2 action (7-A-6), no ESTOP listed for fuel oil purifiers (12-A), and CPP standby hydraulic pump auto start did not have expected results from action input (4-B). Conduct full review of PSTPs Revision 6 dated February 27, 2025 for areas of discrepancies and submit for approval to Class and USCG for review and onboard verification.

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. The #3 auxiliary engine did not start during PSTP testing. Crew identified faulty o-ring on pneumatic starter. Crew replaced o-ring and retested the operation of the #3 auxiliary engine to the satisfaction of the attending MI.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. When testing rescue boat recovery, tube lock limit switch did not operate. Crew applied lubricant and exercised the limit switch. MI retested rescue boat launch and recovery and verified operation of limit switch.

Life-saving appliances shall at all times be in working order, ready for immediate use. 1 Rescue Boat: Secure fuel tanks, batteries not rated for marine service. 2 Lifeboat , batteries not rated for marine service.

Fire protection systems shall be kept in good working order. Cargo fixed water spraying system had several clogged/blocked nozzles.

Measures shall be taken by the Administration to ensure automation equipment is functional and reliable. MIs noted boiler alarms for High & low pressure and salinity are inoperable. Additionally aft safety relief valve is inhibited-needs to be serviced, sight glass missing pin for opening and closing valve.

The fire integrity of division bulkheads shall be maintained. Wire runs and telephone lines are being passed through A class boundary bulkheads without proper insulation. A60 bulkhead MCT from engine room to generator room holed and can see daylight through bulkhead allowing passage of flame and smoke.

The public address system shall be clearly audible above the ambient noise in all spaces. PA not audible IWO of EDG and CO2 rooms.

Where the atmosphere in double hull spaces cannot be reliably measured using flexible gas sampling hoses, such spaces shall be fitted with permanent gas sampling line. Pump room fixed gas monitor inaccurate, reading 21.8 O2 and 4% LEL, verified 20.8 and zero with portable instrument.

The venting arrangements shall be so designed and operated as to ensure that neither pressure nor vacuum in cargo tanks shall exceed design parameter. Ventilation local gauges IWO P/V Valves are out of calibration showing vacuum and inaccurate pressure on cargo tanks.

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Identified 05 exterior fire hoses that were holed/ unserviceable during pressure test.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Vessel experienced a loss of propulsion and blackout due to actions taken after a HT alarm. Master to submit Risk Analysis and Mitigation Plan that includes statement for continued (or suspension) of PUMS continued voyage and mitigation strategy for entry into next port prior of arrival. Determine root cause of alarm condition and the reason for the lower than normal water pressure in HT tank. Make repairs to satisfaction of RO. 74 SOLAS (20) I/11(a)

Vessels ballast treatment system has a chemical corrosion caused leak in a flange preventing the full utilization of the system. Vessel is still able to ballast and deballast as per the masters email. Make repairs to the system as per OEM and restore to full operations.

To assist in casualty investigations, ships, when engaged on in international voyages, subject to the provisions of regulation 1.4, shall be fitted with a voyage data recorder (VDR). It was self reported the the VDR is showing an active alarm code 18 and 22. Prior to the next port make repairs and restore the VDR to full operations. Provide a technician report to RO or USCG to attest to satisfactory operations.

Where davit arms are recovered by power, safety devices shall be fitted which will automatically cut off the power before the davit...During inspection, Marine Inspector found that all three of the limit switches were frozen due to corrosion. Additionally, it was reported that the rescue boat is overweight by 210 kg by attending OEM, lifeboat davit release gear is past due by 58 days per company SMS and rescue boat quarterly drill is also past due. Full functionality per OEM specs is required for the rescue boat and its davit. 74 SOLAS III/34 LSA Code 6.1.2.7

Internal combustion engines of 2250 kw and above having cylinders of more than 300mm bore shall be provided with crankcase oil mist detectors or engine bearing temperature monitors or equivalent devices. Oil mist detection for the main engine is currently showing a fault. This has not caused a slowdown or shutdown of the engine, but system requires repair and to monitor the crankcase for the presence of oil mist. 74 SOLAS II-1/47.2

All lifesaving appliances and arrangements shall comply with the applicable requirements of the code. Each rescue boat launching appliance shall be fitted with a powered winch motor capable of raising the rescue boat from the water with its full rescue boat complement of persons...with stored mechanical energy. 74 SOLAS III/34 LSA Code 17 6.1.1.9

Every person...must receive (1) A minimum of 10 hours of rest in any 24-hour period; and (2) 77 hours of rest in any 7-day period. While auditing work/rest recording software aboard the vessel, Marine Inspector found that several crewmembers had exceeded limits of work/rest hours per 7 days while conducting cargo operations, management reviews, and mooring/unmooring operations. Documentation attesting 07 days of proper rest periods for all crewmembers required prior to sailing. 46 CFR 15.1111(a)

The condition of the ship and its equipment shall be maintained to conform...to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons. Properly terminate all wiring in junction boxes and stuffing tubes. MI’s ID’d exposed wires in the following locations. Under main engine output shaft wet sump tank level indicator, smoke detector over main engine near the exhaust, generator room located ivo blue tank. 74 SOLAS 1-11(a)

Each officer or crewmember who is assigned by the PIC duties and responsibilities related to the cargo or cargo-handling equipment during a transfer of liquid cargo in bulk, but is not directly supervised by the PIC, must hold a tankerman-assistant endorsement. Vessel has an unsupervised Oiler as Pumpman who does not hold a tankerman assistant endorsement. Acting Pumpman carries out unsupervised tasks during cargo transfers without proper training.

Records of daily hours of rest must be maintained onboard the vessel. Each affected person under paragraph (a) of this section must receive a copy of the records pertaining to them, which will be endorsed by the master or by a person authorized by the master and by the seafarer. Engineers are not being tracked. 46 CFR 15.1111(d)

The condition of the ship and its equipment shall be maintained...in all arespects will remain fit to proceed to see without danger to the ship or persons on board. Marine inspectors received notice that the following equipment were not fully operational: start air compressor #1, ship's service air compressor, evaporator. provide evidence that all are operational. 74 SOLAS 1-11(a)

Provisions shall be made to facilitate cleaning, inspection and maintenance of main propulsion and auxiliary machinery. The potable water pumps are short cycling and may need pressure switch replacement or adjustments. SOLAS II-1 26.7

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. the first shot of the Port Anchor chain needs to be replaced due to numerous cracks and unsat previous repairs. SOLAS 1-11(a)

Provisions shall be made to facilitate cleaning, inspection and maintenance of main propulsion and auxiliary machinery. The hydraulic crane motor that is enclosed in the cargo hose crane pedestal has a known leak. The leak has produce approximately 700 liters of hydraulic oil being contained within the pedestal without being rectified. SOLAS II-1 26.7

If the installed BWMS required by this subpart stops operating properly during a voyage, or the vessel's BWM method is unexpectedly unavailable...Marine inspector found that BWMS alarm history showed that, on several occasions over the previous week, the BWMS was not operating properly. Alarms included “Deballasting above MADC”, “BV-300 Purate supply valve fail to close”, and “Ballast pump running without treatment”. Additionally, during a mooring evolution while Marine Inspectors were onboard, the last “Ballast pump running without treatment” ran for over 60 minutes without being answered. Vessel must display proper functionality prior to departure. 33 CFR 151.2040(b)

The company is responsible for ensuring that adequate resources and shore-based support are provided to enable the designated person or persons to carry out their functions. Multiple requisitions initiated by crew are not being addressed by vessel's company. Bridge hand flares expired 01 JUL 24, Lifeboat radio batteries and the BNWAS battery is also expired. Bridge rocket flares, line throwing charges, life boat rocket flares, handflares, SART batteries, and smoke signals expire in SEP 24 and have not been ordered. There is a blatant failure at the company level utilizing NSE. Requisitions are visible within the system however, the company is failing to fulfill requisitions providing necessary supplies to the vessel. The Attending MIs were informed crewmembers that Patriot Shipping Order Staff have also been laid off. Rectify the operational-related deficiencies indicating the failure in maintaining critical items onboard SLNC PAX and share with FEACT for review by 27 OCT 2024. 74 SOLAS (20) IX/3 ISM Code A/3.3 46 CFR 4.05-1

The ship’s Safety Management System (SMS) under the ISM Code (Section 9) mandates that procedures be in place to ensure that non-conformities, accidents, and hazardous situations are reported to the company, investigated, and analyzed with the objective of improving safety and pollution prevention. Attending investigating officer revealed two separate incidents that occurred but were not reported to USCG as required by regulation. 46 CFR 4.03: a marine casualty or accident includes: (ix) Reduction or loss of a vessel's electrical power, propulsion, or steering capabilities. (x) Failures or occurrences, regardless of cause, which impair any aspect of a vessel's operation, components, or cargo. 46 CFR 4.05-1: it is the responsibility of the owner, agent, master, operator, or person in charge to notify the nearest Coast Guard Marine Inspection Office immediately after addressing any resultant safety concerns. 1. The attending Investigating officer has determined that on 20 August 2024, a cargo leak occurred in the cargo pump room involving approximately 4,200 gallons of JP8 fuel into the bilge. This hazardous spill posed a significant environmental and safety risk which should have been immediately reported in compliance with 46 CFR 4.05-1 to the USCG. 2. The attending Investigating officer determined that On 16 August 2024, the ship’s deck log recorded that the ECDIS (Electronic Chart Display and Information System) halted in displaying info. Upon further questioning, the Master stated that the malfunction occurred immediately following a power loss and was restored shortly thereafter. The failure of the ECDIS system coupled with the initiating event the power loss, should have been reported as a marine casualty under 46 CFR 4.03 and 46 CFR 4.05-1. The attending Investigating Officer determined that both events constitute significant hazardous occurrences that should have been reported under the provisions of the ISM Code and US regulations. The failure to report these incidents represents a breach of both regulatory requirements and the vessel’s SMS. Rectify the shipboard procedures addressing reporting methods and notifications to appropriate parties in event of marine casualties by 27NOV2024.

Provisions shall be made to facilitate cleaning, inspection and maintenance of main propulsion and auxiliary machinery. Any discharge into the sea of oil or oily mixtures from ships of 400 gross tonnage and above shall be prohibited except when all the following conditions are satisfied: the ship is proceeding en route; the oily mixture is processed through an oil filtering equipment meeting the requirements of regulation 14 of this Annex the oil content of the effluent without dilution does not exceed 15 parts per million; the oily mixture does not originate from cargo pump-room bilges on oil tankers; and the oily mixture, in case of oil tankers, is not mixed with oil cargo residues. Hydraulic valve actuator system which controls both ballast valves and cargo valves is losing 225 liters of hydraulic oil for every 8-12 hours operated. The location of this leak has not been determined which is concerning as the hydraulic lines run through both the cargo and ballast tanks. The probability exists for the contamination of the aviation fuel which has been delivered to MSC customers or the release of marine pollutants into the sea if the leak is taking place within a ballast tank. The location of this leak needs to be determined and MSC shall be made aware of a potential for recent aviation fuel contamination to protect air assets and crew. SOLAS II-1 26.7 MARPOL I/15.A.2

During inspection, Marine Inspector found that all three of the limit switches were frozen due to corrosion. 74 SOLAS III/34 LSA Code 6.1.2.7

Vessel is restricted from departure from Ulsan South Korea. Vessel is also prohibited cargo operations loading/discharging until further notice. This is based on concerns that have recently arisen following a discussion with MSC and the USCG. Additional information will be forthcoming in the next 24 hours.

Condition of ship shall be maintained for safety to proceed to sea. During MSP Annual Oversight inspection, MIs noted numerous overdue maintenance items for vital systems and below deficiencies. Satisfactory repair plan, with identified location/port for repair/maintenance period, shall be submitted and accepted by FEACT OCMI prior to departure from Singapore. Once approved, ship shall receive authorization to proceed directly for planned maintenance and additional USCG oversight for verification of repairs once complete. 74SOLAS(2020)I/11(a)

When fire-extinguishing medium (CO2) is stored outside a protected space it shall be stored in a compartment with an adequate mechanical ventilation system. MIs noted that force ventilation system for CO2 cylinder room was inoperable. Repairs to be conducted prior to departure. 74SOLAS(2020)II-2/10.4.3

Main and auxiliary machinery essential for propulsion and safety of the ship shall be provided with effective means for its operation and control. MIs noted failure alarms for both #1 and #2 start air compressors. Technicians were onboard attempting to service the equipment but unable to rectify prior to end of inspection. Repairs required for both compressors prior to departure. 74SOLAS(2020)II-1/31.1 25JUN2024: #2 compressor tested sat, #1 compressor (manual mode only, failed PLC for auto mode) - Amended to 15C, manned ER required.

Ships of 500GT and above shall comply with the ISM code for implementation of safety management system (SMS) onboard. ISM(2018) A/7 - Shipboard ops - company should establish procedures, plans, and instructions, including checklists as appropriate, for key shipboard operations. ISM(2018)A/10 - Maintenance of the ship and equipment - Company should ensure that any non-conformity is reported. Ship failed to report inoperable IGS system to USCG. RO was notified, issued condition of class o/a 14FEB2024. RO authorized 2 extensions from FEB2024 to AUG2024 w/out flag state notification or concurrence for inoperable IGS. Flag state requirements: 1. Internal SMS audit due w/in 30 days - root cause analysis into failure from management to notify USCG of IGS failure 2. Revision to cargo operation checklists which detail requirements to ensure IGS operation. As per 74SOLAS(2020)II-2/4.5.5 the subject vessel is not applicable to requirements to have an IGS onboard. Vessel management has elected to decommission the system and shall ensure all requirements are met with RO for system tag out/removal onboard.

Ships of 500GT and above shall comply with the ISM code for implementation of safety management system (SMS) onboard. Maintenance of ship and equipment: Management shall ensure inspections are held at appropriate intervals, appropriate corrective action is taken for non-conformities, and records of these activities are maintained. Ships SMS maintenance tracking system, NSE, was out of date for most of 2024 servicing/repair items and not functional onboard. Vessel and management shall ensure that SMS maintenance tracking system is up to date and accurate onboard prior to resuming cargo operations after maintenance period.

Condition of ship and its equipment shall be maintained. Controllable pitch propeller (CPP) found with a LO sample analysis alert recommending replacement due to high iron content. Additional leaks and blocked alarms for CPP system were noted. Satisfactory repairs/oil replacement w/ maintenance due prior to cargo ops.

Adequate arrangements shall be in place to ensure performance of navigation equipment. Forward anchor light discovered to be inoperable (SMS shows reports as Jan2024 w/ inop). Satisfactory replacement due.

Provisions shall be made to ensure maintenance of main propulsion machinery, to include boilers. Ship's SMS shows overdue replacement of composite boiler feed water valve. 1 of 2 operational and #2 aux boiler has been permanently decommissioned. Satisfactory maintenance item to be completed during planned maintenance period and prior to resuming cargo operations.

Maintenance, testing, and inspection of lifesaving & safety gear shall be carried out. During fire drill, MIs noted SCBA pack leaking. Equipment to be tested and repaired/replaced during maintenance period.

Condition of ship shall be maintained for safety of persons onboard. Ship's general alarm is not audible to personnel in the pumproom. GA system to be repaired/upgraded to ensure adequate coverage for emergency alarms in the pumproom during maintenance period and prior to carriage of cargo.

Condition of ship shall be maintained for safety of operations and environment onboard. #3P (95%) and #4P(98%) high and overfill alarms were inoperable. Satisfactory repairs to restore alarm function required prior to carriage of cargo.

Condition of ship and its equipment shall be maintained for safety of personnel onboard. Oxygen detection system and alarm were inoperable in the pumproom. System function shall be restored prior to cargo ops.

Condition of ship shall be maintained for safety to proceed to sea and cargo operations. The following items were noted during cargo deck & superstructure/house inspection: 1. Port side manifold - Wasted MGO piping and valves corroded 2. #2 Ballast pump temp sensor electrical gland fitting loose 3. #3 Cargo pump pressure sensor fitting leaking cargo and wrapped with electrical tape 4. Electrical junction box (centerline ivo #4S tank) - corroded and holed 5. Fire main support bracked wasted (ivo #4S tank) 6. Port deck wash line bracket wasted (ivo #2P tank) 7. Vent line bracket wasted (ivo #3S tank) 8. Loose bonding strap on FM piping (ivo #2S tank) 9. Electrical cabling supports wasted & cabling unsupported (ivo port centerline fwd tanks). 10. Ballast vent piping heavily corroded throughout cargo deck on P/S sides - several screen missing or dislodged. 11. Wash connection missing bolts (ivo #4P C/T aft tank) 12. 6 sprinker leads clogged & wasted/holed (ivo #4S C/T tank). 13. Deck monitors leaking (Aft Starboard + Fwd) 14. 108 electrical room - wasted fire damper closure 15. Safety gear store room - light out 16. Starboard bridge wing - speaker mount broken Satisfactory repairs due during maintenance period and prior to cargo carriage.

Fire detection systems shall be kept in good working order. MIs noted improper wiring gland w/ loose wires for fire detector installed above steering gear. Satisfactory repairs due during maintenance period and prior to cargo carriage.

Fire protection systems shall be kept in good working order. MIs noted indication lights burned out on watermist main control panel (supply + system available indication lights). Satisfactory repairs due during maintenance period and prior to cargo carriage.

Safety of ship shall be maintained for fitness to proceed to sea. Ship's SMS overdue maintenance items shows overdue main safety system slow down function test. Item shall be completed during planned maintenance period and prior to resuming cargo operations.

Based on PM description, MIs believe this pertains to the OWS 3-way discharge valve. Further investigation by management required to determine system applicability. Test procedure documents must be in a step-by-step or checklist format for required automation testing (PSTPs). They should specify the equipment necessary to perform the tests, alarm set points, and procedures to be followed. MIs noted the following concerns: 1. Inadequate test instructions for SSDGs, Aux Gens, and Boiler safety alarms/shutdowns 2. Missing test equipment (specifically Low LO testing) Revisions required for PSTPs and testing equipment to be provided onboard. OCMI to witness PSTPs during maintenance period and prior to cargo carriage.

Means shall be provided to control the leaks of flammable liquids and ignition sources limited. MIs noted leaks on CPP and ME in engine room. Satisfactory system repairs required during maintenance period and prior to cargo carriage.

Measures shall be taken by the Administration to ensure automation equipment is functional and reliable. MIs noted replacement of seachest valves in lower level engine room. Remote operation for valves is installed ivo ECR (upper ER) and lines to the local valves are disonnected - no remote actuation possible. Remote function shall be restored for seachest valves as system original design. If RO does not require then FEACT OCMI requires documentation to attest to review and RO determination for documentation & acceptance.

Conditions of ship and its machinery shall be maintained. During ER inspection, MIs noted the following: 1. Improper wiring connections and glands ivo [name withheld] coolers (lower lvl ER - fwd) 2. Disconnected level/pressure sensor for CD-P Tank 3. Damaged fuel hose on ME FO gauge display (chaffing) 4. Missing manual/auto switch on sludge pump controller 5. Suspected exhaust leak on ME 6. Replacement of ME air filter 7. Fuel Oil Purifier flat - FO soaked lagging requiring replacement 8. Fuel Oil Purifier flat - inoperable emergency light 9. HFO tank level sensor missing bottom bolt - slight leak 10. Broken pull cable for manual activation of easing gear for composite boiler 11. SSDG #3 blacked out gauge Satisfactory repairs required during maintenance period and prior to cargo carriage.

An alarm system for vital system automation shall be provided. MIs noted 41 blocked alarms for automation & monitoring system onboard. Blocked alarms to be investigated and corrected as applicable during maintenance period and prior to cargo carriage.

The Company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. The safety management system should include procedures ensuring that non-conformities, accidents, and hazardous situations are reported to the company, investigated and analyzed with the objective of improving safety and pollution prevention. The company should establish procedures for implementation of corrective action, including measures intended to prevent recurrence. There is a breakdown of the onboard safety management system based on the number and severity of deficiencies, failure to report, and onboard condition of observed maintenance. Objective evidence includes: vessel has suffered failure of several critical systems and failed to report items to the vessels flag and class in accordance with shipboard SMS procedure PRO-062. Items include failure of port and starboard sea chest valves, OWS, safety MGO relief valve, lube oil purifier, and IGG. Immediate corrective actions to be completed. An external SMC audit required to be completed. 74 SOLAS 20 IX/3.1 ISM Code 18 A/10.1 ISM Code 18 A/9.1-9.2

Each operation described in paragraph 2 of this regulation shall be completed on each occasion whenever any of the following machinery spaces operations take place in a ship. Any failure of the oil filter equipment shall be recorded in the oil record book part 1. On 6FEB2023, a failure was identified in the ships Oil Record Book. Shipboard crew placed an order under REQ#0503658 with the statement “OWS cannot run due to malfunction of PLC and HMI." This requisition was reviewed by the company purchasing manager on 13FEB23. There is no documented records to confirm if the purchase order was created or received by the vessel. A review of the ORB beyond 13FEB23 to the current date identified additional operations on 26JUN23, 18JUL23, 16AUG23 took place but no repairs were ever logged. During the attendance of the USCG on 7-8FEB24, the OWS was inoperable. Interviews with onboard crew indicated that it has been inoperable for roughly a year. There is no evidence to support a notification being made to the RO or OCMI with regard to this equipment. 78MARPOL(2017) II/17.2 and .5

Every candidate for certification in advanced training for oil tanker cargo operations shall be required to: provide evidence of having achieved: the minimum knowledge, understanding and proficiency listed in column 2 of table A-V/1-1-2 and the required standard of competence in accordance with the methods for demonstrating competence and the criteria for evaluating competence tabulated in columns 3 and 4 of table A-V/1-1-2. The C/M is not properly credentialed for chemical tanker operations. STCW Part A V/1-1

For each tankship of more than 5,000 GRT certified for voyages beyond the boundary line as described in part 7 of this chapter — At least two tankerman-engineers must be carried. The 1st A/E does not have a tankerman-engineer endorsement in accordance with table 2 to 46 CFR 15.860(a). 46 CFR 15.860(b)(3)

The ship is equipped with three air compressors, only two of which are currently working. Currently, the start air is cross connected to the working air and the control air to maintain operations. 74 SOLAS 20 I/11(a)

The company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. The subject vessel has an established SMS procedure for the use of NSE, but the inventory onboard doesn’t match what is on hand, including critical spares. ISM Code 18 A/7.1

Exposed metal parts of electrical machines or equipment which are not intended to be live but which are liable under fault conditions to become live shall be earthed. Bonding wires are missing or damaged throughout the cargo deck, deck lighting is showing signs of environmental intrusion within the cargo deck, and engineering space packing glands to numerous motors, controllers and lights are loose, and the fire detection head in the bosun hold is also loose. In addition, numerous junction boxes are compromised. 74 SOLAS 14 II-1/45.1.1

The cargo deck tanks are being utilized for the storage of MGO and transferring to the engine room using the F/O stripping line without an approved fuel oil transfer plan. 74 SOLAS 20 I/11(a)

The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose, the following functional requirements shall be met: means that’ll be provided to control leaks of flammable liquids. Numerous fuel and lube oil leaks noted by MI, including F/O transfer pump No.1 and L/O transfer pump. CPP system oil leaking under both pumps (two catch trays are in place). Main Diesel Engine aux oil pump containment is full of oil. Lube oil purifier feed pump is leaking oil. 74 SOLAS 14 II-2/4.1

In addition to the fire protection systems and appliances listed in paragraph 2.2.3, tankers shall have a maintenance plan for inert gas systems and deck foam. Attending MI’s noted that the IGG system shares a salt water line to the scrubber with the ballast system. This shared s/w connection prevents the inert gas generator and the ballast system from operating simultaneously. Additionally, the s/w supply to the IGG is leaking through wasted piping into a void space, which has caused additional equipment damage. Also, attending MI’s noted that the low expansion foam utilized for the cargo deck is past it annual testing requirements. 74 SOLAS 14 II-2/14.4

An efficient bilge pumping system shall be provided, capable of pumping from and draining watertight compartments under all practical conditions. Numerous flexible hoses were observed tied into the installed bilge system and through the engineering spaces and are being used for management of bilge water instead of using the installed systems. 74 SOLAS 14 II-1/35-1.2.1

A means of communication shall be provided between the navigation bridge and the steering gear compartment. The sound powered phone within the steering gear space will not allow for two way communication with the bridge. 74 SOLAS 14 II-2/29.8.10

Numerous gauges throughout the engine room are inoperable, missing, and/or incorrectly mounted, including but not limited to the L/O feed pump, industrial pump, bilge pump, and sea water pump. 74 SOLAS 20 I/11(a)

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations. Attending MI’s noted that the EDG exhaust is leaking at the turbo. 74 SOLAS 20 I/11(a)

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations. Restore the boiler gauge glass for the composite boiler and install the required handles for opening and closing. 74 SOLAS 20 I/11(a)

The company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. Oil analysis system is showing an alert in the CPP oil system and urgent for the deck slewing crane. Complete advised actions as per company SMS procedures. ISM Code 18 A/7.1

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations. Repair ship's service and control air reducing stations. 74 SOLAS 20 I/11(a)

Each officer or crewmember who is assigned by the PIC duties and responsibilities related to the cargo or cargo-handling equipment during a transfer of liquid cargo in bulk, but is not directly supervised by the PIC, must hold a tankerman-assistant endorsement. The 2/M does not hold the required endorsement on credential. 46 CFR 15.860(f)(4)

Main and auxiliary machinery essential for the propulsion and safety of the ship shall be provided with effective means for its operation and control. MIs noted several missing valve handles from vital system piping controls. Valve handles required to be installed. 74SOLAS(2014)II-1/31.1

An efficient bilge pumping system shall be provided, capable of pumping from and draining any watertight compartment. MIs noted missing hardware from bilge suction piping for starboard fwd engine room. Installation shall be corrected to restore suction protection. 74SOLAS(2014)II-1/35-1.2.1

Condition of ship and its equipment shall be maintained for safety and fitness to proceed to sea. MIs noted inoperable lube oil purifier. Equipment shall be repaired/replaced to restore satisfactory function. 74SOLAS(2020)I/11(a)

Ventilation of machinery spaces shall be sufficient under normal conditions to prevent accumulation of oil vapor. MIs noted clogged and dirty engine room ventilation screens impacting sufficient ventilation. Ventilation screens shall be cleaned and restored to free condition prior to departure. 74SOLAS(2014)II-2/4.2.2.2

Terminations and joints in all conductors shall be so made as to retain the original electrical, mechanical, flame-retarding and, where necessary, fire-resisting properties of the cable. MIs noted improperly terminated wires in the engine room and generator spaces. Terminated wiring shall be properly removed or connections properly made. 74SOLAS(2014)II-1/45.5.6

A chemical tanker shall comply with the requirements of the International Bulk Chemical Code. After any survey of the ship for compliance with the IBC code, no change shall be made in the structure, equipment, fittings, arrangement, and material covered by the survey without sanction by the Administration. Current active Certificate of Fitness (CoF), issued by ClassNK (RO), is incorrect and authorizes heat cargo carriage; Heating coils in cargo tanks are current inoperable and no longer used. RO to update vessel details and cargo limitations in issued CoF. 74SOLAS(2014)VII/10.1 IBC(2016)1.5.3.2

Oil and chemical tankers of 500 GT, to include the company, shall comply with the requirements of the International Safety Management Code (ISM). The company should establish procedures to ensure that the ship is maintained. In meeting this requirement, the company should ensure that: 1. Inspections are held at appropriate intervals; 2. Any non-conformity is reported, with its possible cause, if known; 3. Appropriate corrective action is taken. MIs noted 70+ overdue maintenance tasks in the vessel's SMS system for engineering systems. Overdue tasks shall be completed and effectiveness of SMS evaluated onboard by RO (ABS) for ISM compliance. 74SOLAS(2020)IX/2.1.2 ISM(2018)A/10.1 ISM(2018)A/10.2

Hydraulic power-operated steering gear shall be provided with the following: 1. Arrangements to maintain cleanliness of hydraulic fluid 3. a fixed storage tank having sufficient capacity to recharge at least 1 power actuating system including the reservoir, and shall be permanently connected by piping such that the hydraulic systems can be readily recharged. MIs noted leaking hydraulic oil from steering pumps, and missing valve handle off of the fixed storage tank for hydraulic reservoir. Fix prior to departure. 74SOLAS(2014)II-1/29.12

Emergency escape breathing devices (EEBDs) shall comply with the Fire Safety Systems Code (FSS) and spare devices shall be kept onboard. MIs noted EEBD ivo boiler in engine room that had zero pressure. EEBD to be replaced with satisfactory one. 74SOLAS(2014)II-2/13.3.4

A general emergency alarm system shall be provided. The system shall be supplemented by a public address system. The system shall be audible throughout all the accommodations and normal crew working spaces. MIs noted that the GA/PA system was not audible in spaces throughout the accommodations and on the cargo deck. Satisfactory repairs and speakers required to ensure adequate coverage of the GA/PA system. 74SOLAS(2014)III/6.4.2 74SOLAS(2014)III/6.4.3

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. MIs noted FRB with exhaust leak. Satisfactory repairs required. 74SOLAS(2014)III/20.2

MIs noted limit switch on rescue boat davit preventing tube locking and launching operations. Life-saving appliances shall be in working order and ready for immediate use. Rescue boat throttles installed incorrectly, resulting in boat going opposite of throttle labeling (ahead/astern switched).74 SOLAS (20) III/20.2

Means shall be provided to control leaks of flammable liquids. The main source of electrical power shall consist of at least two generating sets. The SSDG #3 has significant fuel oil leak on fuel return line, exhaust leak, and jacket water leak on exhaust manifold at plug. SSDG #2 and #3 generator wire casing are missing bolts and are not secured. 74 SOLAS (04) II-2/4.1.1

Oil tankers of 150 gross tonnage and above shall be equipped with an oil discharge monitoring and control system. The ODME is not operating as designed a fails self-test with error codes for “wiper” (sensor), fresh water, and air supply.73/78 MARPOL Annex I/31

Oil filtering equipment installed in accordance with 73/78 MARPOL Annex I/14.1 shall be such as will ensure that any oily mixture discharged into the sea after passing through the system has an oil content not exceeding 15 ppm. OWS was inoperable and unable to be tested.73/78 MARPOL Annex I/14.6

Electrical equipment shall be of a certified safe type for use in the dangerous zones to which it may be exposed. The explosion proof lights in the paint locker have cracked covers and light on the main deck at the entrance to the paint locker has evidence of water intrusion.74 SOLAS (04) II-2/19.3.2

Exposed air pipes to ballast and other tanks shall be of substantial construction and shall be provided with automatic closing devices. Multiple ventilation heads on deck are wasted, missing flame screens, or are damaged including the forward “4P WBT,” starboard lube oil tanks ventilators on starboard quarter, “5S WBT.” ILLC (88) Annex I/Reg 20

Fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs including structural fire protection and fire-resisting divisions. Engine Room Casing door “509” does not latch shut and appears to be broken as centerline bows out while closing. 74 SOLAS (04) II-2/14.2

The company should establish procedures, plans, and instructions for key shipboard operations. Test equipment provided is missing or inadequate to conduct periodic safety test procedures. Hand pump is missing required gauges for pressure indications for use during testing. 74 SOLAS (20) IX/5 ISM Code Clause 7

Cables and wiring shall be installed in such a manner as to avoid chafing or other damage. General Service/ Fire pump power cables into control box are missing stuffing glands. Wires are missing insulation entering “No. 5 Group Starter Panel” distribution panel. 74 SOLAS (04) II-1/45.5.5

Machinery shall be adequate for the service for which it is intended and the company should ensure adequate shore based support and appropriate corrective action is taken. Working air compressor has multiple oil leaks on fittings and hoses, standing oil in interior and exterior containment around compressor, with oil-soaked sorbent pads present. Compressor fan is covered in oil and dirt. “Oil Change Due” is indicated on control panel. A replacement air compressor was removed from the December 2022 shipyard specification and these issues were documented on CG-835 issued 16Jan23 with no corrective actions taken.74 SOLAS (04) II-1/26.1 and 74 SOLAS (20) IX/5 ISM Code Clause 7 and Clause 10.2.3

Cables and wiring shall be installed and supported in such a manner as to avoid chaffing or other damage. Junction box for light 5L6 in lower Engine Room is not mounted. 74 SOLAS (04) II-1/45.5.5

The company should establish procedures, plans, and instructions for key shipboard operations. Periodic safety tests required by 46 CFR 61.40-6 (see NVIC 01-13, Ch. 1) must demonstrate the proper operation of the primary and alternate controls, alarms, power sources, transfer override arrangements, interlocks, and safety controls. Systems addressed must include fire detection and extinguishing, flooding safety, propulsion, maneuvering, electric power generation and distribution, and emergency internal communications. Revision 4A has been approved for testing with the OCMI by Marine Safety Center but required testing has not been conducted for final approval. 74 SOLAS (20) IX/5 ISM Code Clause 7 ISM Code 3.3

The condition of the ship shall be maintained. The piping connecting the IG main to the PV breaker is corroded. Clean, gauge, and recoat or replace.74 SOLAS (20) I/11(b)

Access openings in bulkheads at ends of enclosed superstructures shall be weathertight when closed. Weathertight door to BWTS chemical room is installed upside down so dogs fall to open position. This item was previously identified in July 2022 and remains improperly installed.ILLC (88) Reg. 12

Every tanker shall be provided with the means to enable the crew to gain safe access to the bow in accordance with MSC.62(67), Revised Guidelines for Safe Access to Tanker Bows, which requires access to be by means of either a walkway on the deck or permanently constructed gangway which should be constructed of fire resistant and non-slip material. The type approval certificate for the FRP deck grating installed on deck, raised walkways, and the cargo manifold area states “not to be used as walkways or in areas, which may be used for escape or access for firefighting, emergency operation or rescue in cargo holds and open decks or semi-enclosed areas” under ‘service restriction.’ 74 SOLAS (04) II-2/3-3.2

74 SOLAS (20) II-2/14.2 and ISM Code Clause 7 Fire-fighting appliances shall be kept in good working order and the company should ensure appropriate corrective action is taken to maintain the ship in conformity with provisions of relevant rules and regulations. The main fire pump is inoperable and tagged out on the main switchboard. NSE WO/SR No. 0517709 titled “Replace Fire & G/S Pump” dated June 4, 2022 indicates replacing the fire pump was to be scheduled for drydock on October 1, 2022 and was ordered per Requisition 0503411. The fire pump replacement has not been installed as of 16Jan23. Additionally, provide copy of notification made to RO (ClassNK) and USCG and provide SMS procedures for reporting marine casualties.

74 SOLAS (04) II-1/26.1 Machinery shall be adequate for the service for which it is intended. The WB Pump #2 has an oil leak from either the gas-tight bulkhead seal or bearing.

74 SOLAS (04) II-1/26.1 Machinery shall be adequate for the service for which it is intended. Cargo Pump #3 has not been proven operational after maintenance and reinstallation. Provide service report and post-maintenance pressure test data.

74 SOLAS (20) IX/5 ISM Code Clause 7 and Clause 10.2.3 The company should ensure appropriate corrective action is taken to maintain the ship in conformity with provisions of the relevant rules and regulations. Independent high and overfill alarm system is not operating as designed and alarms are non-operational. The audible alarm on deck was unable to be demonstrated. The radar gauging system for cargo tanks 2P and 4P/S slops tank remain inoperable. Cargo valve actuators in COT 2P and 3P are inoperable. Port slop tank valve “COV009” has a bad solenoid. The ballast tank pressure sensors in WBT 2S, 4P, and 4S remain inoperable. Provide service reports.

74 SOLAS (20) IX/5 ISM Code Clause 7 and 74 SOLAS (20) VII/9 IBC 3.6 The company should establish instructions for key shipboard operations and provisions shall be made for the distinctive marking of pumps, valves, and pipelines to identify the service and tanks which they serve. Cargo pumps and ballast pumps, and their associated gauges, are not labeled in the Cargo Pump Room.

74 SOLAS (04) II-1/26.1 Machinery shall be adequate for the service for which it is intended. Jacket water pre-heater selector switch is missing and heater cannot be operated locally.

Critical gauges and calibration intervals and procedures are not identified in company SMS, no standard job exists in NSE for calibration of gauges, and records of calibration available indicate no calibrations more recent than 2019.

74 SOLAS (04) II-1/26.1 Machinery shall be adequate for the service for which it is intended. The following gauges in the engine room are in need of repair (not all inclusive): - Gauge labeled “Unit Outlet” in purifier space on fuel skid is being held together, or held in place, by zipties

The following gauges in the cargo pump room are in need of replacement (not all inclusive): - Gauges labeled with handwritten blue “1” (set of two) the left gauge is missing approximately 30% of case fill and the right gauge is missing 75% of case fill. - Gauges labeled “[?] For Cargo Pump III” (set of two) the right gauge is missing 30% of case fill and is wrapped with cloth for unknown reason. The following gauges in the cargo pump room are in need of repair (not all inclusive): - Gauges labeled with handwritten blue “1” (set of two) the right gauge is not secured and is being connected to the supply line with zipties. - Gauges labeled with handwritten pink “1” (set of two) the right gauge is secured with zipties. - Gauges on lower level port bulkhead (set of two) the right gauge is not secured.

74 SOLAS (04) II-1/26.1 Machinery shall be adequate for the service for which it is intended. The MDE turbocharger installed is not adequate for MDE operational range. While original installation, during operational testing, the turbocharger appeared starved of air and exceeding operational parameters at 78% of MDE capability. Provide servicing report.

74 SOLAS (20) V/19.2.7 and 74 SOLAS (20) IX/5 ISM Code Clause 3.3 All ships over 3,000 GT shall have a 9GHz and a 3GHz radar and the company should ensure adequate shore-based support and appropriate corrective action is taken. NSE WO/SR No 0517859 titled “Radar X-Band Check” dated June 28, 2022 conducted by the C/O indicates the X-band radar console has “issues: display very dim and hard to read during the day. Tune will drop at times especially during anchor watch. Will at times show wrong range/bearing of targets. Only fix is to reboot radar.” Marine Inspector witnessed S-Band radar screen take hours to display properly with words and numbers being illegible and lines being distorted. Replacement radars were removed from the December 2022 shipyard specification.

74 SOLAS (20) III/20.3 Maintenance, testing and inspections of life-saving appliances shall be carried out in a manner having due regard to ensuring reliability of such appliances. Extensive corrosion is evident on rescue boat davit. The December 2022 yard specification indicated the rescue boat davit needs to be pressure washed and coating renewed. The five yearly servicing was completed and many wasted components were replaced but the blasting and coating were not completed.

74 SOLAS (20) IX/5 ISM Code Clause 7 and Clause 10.2.3 The company should establish procedures, plans, and instructions for key shipboard operations. Maximum Allowable Working Pressure (MAWP) for cargo system not indicated in SMS. Approved drawing M1-4331 “Cargo Loading/Unloading & Stripping Sys” indicate working pressure is 10 bar. Provide SMS document outlining MAWP and 1.5x test pressure with cargo piping hydrostatic test certificate. Alternately, provide cargo piping hydrostatic test certificate with test pressures in accordance with parameters on approved drawings.

The machinery shall be adequate for the service for which it is intended. After several hours of running, No. 1 Diesel Generator was reported to shutdown without notice while under load.

The ship and its equipment shall be maintained. The cargo control monitoring system is not operating as designed: 1- Bjorge monitoring and control system console 'X102' is inoperable due to CPU failure. Discharge pressures and bearing temperatures are unavailable in the cargo control room and must be taken manually at manifold or on console 'X101' in engine control room. 2- the independent radar gauging system computer shows unidentified intermittent alarm for cargo tank 3P indicating 98-99% full. Standing alarm dated 07July2022 while the tank was empty and gas-free. 3- ECR Bjorge "Cargo Tank" screen indicates all cargo tanks and 'Deck Tank S' are full while all cargo tanks are empty and gas-free. 4- high and overfill alarms for 'Slop Tank S' were found inhibited due to faulty sensor in need of repair. Engine Control Room and Cargo Control room shall be manned for the duration of transfer operations and adequate communication ensured. 74 SOLAS I/11(a) MARPOL Annex I/6.4.1

The company and the ship shall comply with the requirements of the International Safety Management code, to include establishment of procedures to ensure the ship is maintained with regulations & appropriate corrective action is taken upon non-conformity reports. The following cargo systems non-conformities were reported with no follow up from the company. 1-Cargo pump #3 mechanical seal leak- work order (SR 0515190) open since NOV 2021, with "near miss" report for hazardous condition filed by ship's Captain on 14MAY2022 with no rectification or servicing arrangements by management. Cargo pump #3 is prohibited from use until approved repairs. 2-Pump room bulkhead hyd penetration leaks from fittings reported via 4 open work orders & report from 19APR2022. 3-Alarm intermittent issue with overfill audio alarms on deck & corroded speaker system and supporting bracket failing submitted 01MAY2022. Immediate corrective actions to be completed. External SMC audit required to be completed focused on ISM maintenance and procedures & corrective actions implementation by company. CITE: 74 SOLAS (2020) IX/3.1 ISM Code (2018) 10.1 ISM Code (2018) 10.2.3 Updated 17JUN2022: Company appeal to OCMI regarding external SMC audit requirement was granted in part. Requirement for additional external SMC audit removed. Flag instructions to be provided to ABS regarding maintenance/corrective action implementation by management as focus areas for upcoming intermediate SMC audit. FEACT MIs to attend audit. Secondary appeal for extension of material condition repairs granted, items extended to 24JUL2022 for final repair in Busan, ROK. Update 23JUN2022: Extended to 24AUG2022 for final repair in Busan, ROK. Pump restricted from use until repaired and verified by ClassNK.

Conditions of the ship shall be maintained to ensure it remains fit for sea without danger to it or persons onboard. Marine Inspector noted substantial corrosion & wastage to sprinkler piping, electrical conduit piping for control/sensor cabling, and air lines in way of cargo tank 3P hatch. Conduct repairs to suspect & defective piping. CITE-74 SOLAS (2020) I/11(a)

Ventilators passing through superstructures shall have substantially constructed coamings of steel. Marine Inspectors noted corroded deck vent, port side in the changing/laundry room. Conduct satisfactory repairs to ventilator. CITE - ICCL (2005)19(2)

Performance of navigational equipment shall be maintained. Magnetic compass experiencing deviations in excess of 5 degrees at multiple headings. Conduct satisfactory servicing/adjustment of compass. CITE - 74 SOLAS (2020) V/16.1

Maintenance, testing and inspections of life-saving appliances shall be carried out in a manner with due regard to ensure reliability of such appliances. Rescue boat search and position lights are not wired IAW schematic, fuse is bypassed. Conduct repairs IAW approved drawings. CITE - 74 SOLAS (2004) III/20.3

Each oil-fired boiler shall have safety arrangements which shut-off the fuel supply or flame failure. Composite boiler does not shut-off on flame failure. CITE - 74 SOLAS (2004) II-1 Reg 32.2

Machinery and associated pipings systems, and fittings shall be of a design and construction adequate for the service which they are intended. The following items were noted by Marine inspectors: 1-Two pipe flanges on the Main Engine Lube Oil cooler system found leaking. 2-Discharge elbow on the Inert Gas System raw water pump is holed from corrosion. 3-Failed Aux Salt Water supply pipe to Evaporator has been replaced with flex hose and hose clamps. CITE - 74 SOLAS (2004) II-1/26.1

Fire-fighter’s outfits shall comply with the Fire Safety Systems Code - Personal equipment shall consist of the following: axe with a handle provided with high-voltage insulation. All four fire fighter outfits did not have an axe as required. 74 SOLAS (14)II-2/10.1.1 FSS Code 2.1.1

The automatic stopping device is a device used, where applicable, to automatically stop any discharge overboard of oily mixture when the oil content of the effluent exceeds 15 ppm. The automatic stopping device should consist of a valve arrangement installed in the effluent outlet line of the 15 ppm Bilge Separator which automatically diverts the effluent mixture from being discharged overboard back to the ship’s bilges or bilge tank when the oil content of the effluent exceeds 15 ppm. The valve arrangement installed to automatically divert the effluent mixture overboard or back to the ships bilges does not function rendering the oil filtering equipment inoperable. Repair or replace with new, and test to the satisfaction of Class. MARPOL Annex I/14.7 MEPC 107(49)/3.6

After any survey of the ship under regulations 7, 8, 9 or 10 has been completed, no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without the sanction of the Administration. The foam service and general service fire pumps priming system are not installed as manufactured and inoperable. Install in accordance with manufacturer’s procedures and test to the satisfaction of Class. 74 SOLAS (14)I/1.b

The engine shall be capable of operating when the lifeboat is flooded up to the center line of the crankshaft. During underway operations, the rescue boat experienced excessive smoke in the engine compartment because of engine contact with accumulated water in the bilge that was well below the center line of the crankshaft. 74 SOLAS (14)III/34 LSA Code 4.4.6.4

Cables and wiring shall be installed and supported in such a manner as to avoid chafing or other damage. Wasted wire supports for electrical cables located on the starboard side house exterior directly below and in way of rescue boat deck. Sections of the electrical cables are detached from the exterior bulkhead. Replace wire supports with new to the satisfaction of Class.

Means shall be provided to drain all cargo pumps and all oil lines at the completion of cargo discharge, where necessary by connection to a stripping device. The line and pump draining shall be capable of being discharged both ashore and to a cargo tank or a slop tank. For discharge ashore a special small diameter line shall be provided and shall be connected outboard of the ship's manifold valves. No. 2 cargo pump stripping line in cargo pump room discovered with unapproved soft patch. Prior to use for stripping operations, make permanent repairs to line and test to the satisfaction of Class.

74 SOLAS (04) Chapter II-1/Part A-1, Reg.3-1, ICLL66 Reg. 12: In addition to the requirements contained elsewhere in the present regulation, ships shall be designed, constructed and maintained in compliance with the structural, mechanical, and electrical requirements of a classification society. WTD 48 fails to close properly to prevent water from entering space and hosptal space WTD with wastage. Replace with new/repair to the satisfaction of Class.

74 SOLAS (04) Chap II-1/43.2: The electrical power available shall be sufficient to supply all those services that are essential for safety and in an emergency, due regard being paid to such services as may have to be operated simultaneously. The emergency generator jacket water was leaking excessively upon start up at the engine pre-heater. Repair jacket water leak and prove proper operation to the satisfaction of the Coast Guard.

74 SOLAS(09) Chapter II-2/14.2.1.1: The following fire protection system shall be kept in good order so as to ensure their required performance if a fire occurs. The sprinkler heads in the IGG room was clogged with debris. Remove sprinkler heads and clear debris to the satisfaction of the Coast Guard.

74 SOLAS(09) Chapter II-2/5.2.1.1: The main inlets and outlets of all ventilation systems shall be capable of being closed from outside of the spaces being ventilated. The means of closing shall be easily accessible as well as prominently and permanently marked. The ventilation closures for emergency diesel generator room, inert gas generator room, crew lounge, and galley vents were seized and corroded. The galley vent screen on aft poop deck was wasted. Repair and demonstrate proper operation of ventilation closures.

74 SOLAS(09) Chapter II-2/4.5.5.1.2, FSS Code Reg 15 Paragraph 2.4.3.1: Tankers operating with a tank cleaning procedure with a tank cleaning procedure shall be fitted with an inert gas systems complying with the Fire Safety Systems Code and with fixed tank washing machines. For inert gas systems of both the flue gas type and inert generator type, audible and visual alarms shall be provided. The IGG alarm panel in Cargo Control Room did not alarm during auto shut down tests. Repair and demonstrate proper operation of IGG alarms.

74 SOLAS (09) Chapter II-2/4.5.5.1.2, FSS Code Reg 15 Paragraph 2.1.2: Tankers operating with a tank cleaning procedure with a tank cleaning procedure shall be fitted with an inert gas systems complying with the Fire Safety Systems Code and with fixed tank washing machines. The inert gas systems refer to Chapter II-2 of the Convention shall be designed, constructed and tested to the satisfaction of the Administration. The IGG flame window glass is cracked. Replace glass to the satisfaction of Class.

74 SOLAS (04) Chapter II-1/40.1.3: Electrical installations shall be such that the safety of crew and ship from electrical hazards will be ensured. Multiple dead end wire runs from the upper engine room to the lower engine room between cargo pumps No. 1 & 2. Remove dead end wire to the satisfaction of Class.

74 SOLAS (04) Chapter II-1/3-1: In addition to the requirements contained elsewhere in the present regulation, ships shall be designed, constructed and maintained in compliance with the structural, mechanical, and electrical requirements of a classification society. The air diaphragm pump discharge gauge in cargo pump room lower flat is broken and unserviceable. Replace with new in-kind gauge to the satisfaction of Class.

74 SOLAS(04) Chapter II-1/40.1.3: Electrical installations shall be such that the safety of crew and ship from electrical hazards will be ensured. Found all electrical motors grounding cables in lower engine room not grounded or missing.

74 SOLAS (04) Chapter II-1/46: Every ship shall be provided with documentary evidence, to the satisfaction of the Administration, of its fitness to operate with periodically unattended machinery spaces. PSTP for 12-B-1 Remoted Fuel oil Valves shutoffs are incorrect. The fuel valves group and number on PSTP are missing. The placard for the remote fuel valves group outside the engine room does not match with the local fuel valves group down in the engine room. Prove proper test procedure to the satisfaction of Class.

74 SOLAS (04) Chapter II-1/3-1: In addition to the requirements contained elsewhere in the present regulation, ships shall be designed, constructed and maintained in compliance with the structural, mechanical, and electrical requirements of a classification society. The main engine’s oil leak off piping behind the governor was disconnected and rigged with a hose, reclaiming the engine oil back into the main engine sump through the sounding tube. Vessel shall provide procedure to Class for review and determination.

74 SOLAS (04) Chapter II-1/46: Every ship shall be provided with documentary evidence, to the satisfaction of the Administration, of its fitness to operate with periodically unattended machinery spaces. PSTP for 12-A-1 Remote Electrical Shutoffs for Fans and Pumps are incorrect. The electrical breakers listed on the PSTP does not match the actual electrical breakers on the switchboard and electrical breaker reset sheet. Prove proper test procedure to the satisfaction of Class.

74 SOLAS(04) Chapter II-1/3-1: In addition to the requirements contained elsewhere in the present regulation, ships shall be designed, constructed and maintained in compliance with the structural, mechanical, and electrical requirements of a classification society. The deck in way of provision WTD 428 severely wasted with holes. Repair to the satisfaction of Class.

74 SOLAS (04) Chapter II-1/3-1: In addition to the requirements contained elsewhere in the present regulation, ships shall be designed, constructed and maintained in compliance with the structural, mechanical, and electrical requirements of a classification society. Handrails with wastage and holes behind emergency engine room and aft stairwell in poop deck. Repair to the satisfaction of Class.

74Solas(04) Chapter II-1/3-1 In addition to the requirements contained elsewhere in the present regulation, ships shall be designed, constructed and maintained in compliance with the structural, mechanical, and electrical requirements of a classification society. Port and starboard cargo manifold stanchion support base is severely wasted. Replace to original and to the satisfaction of Class.

74 SOLAS (04) Chapter II-1/3-1: In addition to the requirements contained elsewhere in the present regulation, ships shall be designed, constructed and maintained in compliance with the structural, mechanical, and electrical requirements of a classification society. Port and starboard side Pilot’s embarkation safety chains are severely wasted and unusable. Replace to original and to the satisfaction of Class.

74 SOLAS (09) Chapter II-2/7.1: The purpose of this regulation is to detect fire in the space of origin and to provide for alarm for safe escape and fire-fighting activity. The optical smoke detector in the hospital space is inoperable. Repair and prove proper operation to the satisfaction of Class.

The placard sign for the Cargo Pump Room was missing and could not be found by the crew. The CG-835V, will be revised to OSHA standards for shipboard operations per Title 29, Code of Federal Regulations, Part 1915.16, Subpart B: The Employer shall ensure that each sign or label posted to comply with the requirements of this subpart is presented in a manner that can be perceived and understood by all employees,

MARPOL Annex 1. Chapter 2 Reg 6.4: An annual survey within three months after each anniversary date of the Certificate including the general inspection of the structure, equipment, systems, fittings, arrangements and material refer to (1.1) of this regulation to ensure that they have been maintained in accordance with paragraphs 4.1 and 4.2 of this regulation and that they remain satisfactory for the service which the ship is intended. Found discrepancy on the IOPP for bilge tank capacity did not match Tank Capacity Plan DWG Number B1-0301. The IOPP indicates 9 m3 and Tank Capacity Plan drawing states 29.7 m3. Required verification from Class Society.

74 SOLAS (14) Chapter III/20.2: Before ship leaves port and at all times during the voyage, all lifesaving appliances shall be in working order and ready for immediate use. The free fall life boat beacon light is inoperable and the engine fuel filter maintenance was not completed. Replace inoperable beacon light with new and complete life boat maintenance to the satisfaction of Class.

74 SOLAS (09) Chapter II-2/15.2.4: General arrangement plan shall be permanently exhibited for the guidance of the ships officers clearly for each deck control stations, the various fire sections enclosed by A divisions, the sections enclosed by B divisions together with particulars of the fire detection and fire alarm systems. The fire control symbols in galley for fire dampers are faded and unreadable. Replace with new fire dampers placards in accordance with the approved fire control plan to the satisfaction of Class.

74 SOLAS (14) Chapter III/20.7.2: Inspection of lifesaving appliances, including lifeboat equipment, shall be carried out monthly using the checklist required by regulation 36.1 to ensure that they are complete and in good order. The rescue boat’s portable fire extinguisher is severely wasted and unserviceable. Replace with new in-kind portable fire extinguisher.

74 SOLAS(04) Chapter II-1/26.1: The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which it is intended and so installed and protected as to reduce to a minimum and danger to persons onboard due regard being paid to moving parts, hot surfaces and other hazards. The number one generator’s’ jacket water hose was cracked and dry rotted. Replace jacket water hose to the satisfaction of the Coast Guard.

46 CFR 147.60: Cylinders must always be secured when not in use. They must be stored in the upright position with the valve protection cap in place. Lockers and housing must be vented to the open air near the top and bottom for positive circulation of vapors. Cylinders must be protected all sources of heat which may cause the cylinders to be heated to a temperature higher than 130 degree Fahrenheit. Acetylene compressed bottles stowed in the IGG fan room, an unapproved storage for compressed gases. Relocated compressed bottles to a designated locker or housing for proper stowage.

Approved Periodic Safety Test Procedure (PSTP):Complete review and submit for approval to Marine Safety Center (MSC) automation test procedure that demonstrates proper operation of machinery to include testing procedures & set points. After satisfactory review by MSC PSTP must be verified and approved by OCMI prior to vessel operating either PUMS or MAMS.

Several bonding straps broken off in pump room and throughout vessel.

Submit a permanent repair proposal to this office and the repair proposal must be approved prior to conducting permanent repair on Starboard Slop Tank and boundary with No. 4 WBT.

Identify and submit a monitoring system on the clad welded area as well as other areas noted for heavy pitting in the bottom plating and the lowest portion of the corrugated bulkhead.

"Fire Hazard-Cargo pump room: Upon entry into the cargo pump room, strong benzene odor was observed. Upon closer exam, it was discovered the deck is saturated with product. Observed #3 cargo pump leaking product at approximately 5 drops per second. Asked Chief Mate to test air quality with dragger and Benzene was identified. See Narrative for full deficiency.

Routine maintenance items of deck and machinery equipment are overdue as per ships computerized maintenance program (NS5). Over 60 items identified on the Machinery side…deck side estimated to have 40 items. Complete external audit of the vessel SMS and the vessel maintenance program regarding the overdue items. Audit shall be completed to the satisfaction of the OCMI

Approved Periodic Safety Test Procedure. Complete review and submit for approval automation test procedure to demonstrate proper operation of machinery to include testing procedures and set points. Multiple systems to include the boiler and steering could not be tested to the current PSTP.

EIAPP certificate for the SDG is missing and could not be confirm. Provided EIAPP cert of the three generators on board.

PSTP must demonstrate proper operation of the test. Boiler safety on repeater panel could not be demonstrated on the bridge or any other stations. Technician worked on the systems 2 days earlier and system is now inoperative. Complete repair to the boiler safety and provide technician report. Engine room watch shall be fully manned until proper operation of the boiler safeties can be demonstrated.

Make repair to the A-60 boundary IWO engine room emergency escape trunk.

Lighting in the E/R escape trunk is inoperable…02 bulbs are blown. Replace light broke light bulbs and complete to the satisfaction of the OCMI.

Fire hazard: Bilges contain extensive amounts of oil from main and auxiliary machinery. Bilge coated with long standing dirt/oil mixture coming from SSDE and MDE; oil soaked rags & debris in bilges well under engines. Clean excess oily bilge in the E/R.

Engine space-Purifier Room: Excess oily deck, oily rags and oil soaked lagging presents a fire hazard. Clean purifier room to the satisfaction of the OCMI.

Engine space-Purifier Room-fire hazard: Replace oil soaked lagging/insulation to the satisfaction of the OCMI.

Replace oil soaked fire extinguisher in the E/R at station 41. Replacement extinguisher shall be USCG approved.

Repair side laundry room dryer vent to allow unobstructed passage of air.

The vessel is not sufficiently manned to maintain satisfactory material condition, sufficient maintenance and effective security.
