The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Vessel had experienced three losses of propulsion, while attempting to engage astern propulsion in the last 60 days. Repairs have been previously made; however, the issue still persists. Currently, there are no short and long term means to prevent future failures of this type of propulsion malfunction. SOLAS 04 I/11. 16b.
