Resolution: Class surveyor attended vsl and L/O lines were tighted to #2 DG, and DG tested with no leaks present. General cleanliness of engine room was improved and cleaned.

The construction of doors and doors frames in "A" class divisions, with the means of securing them when closed, shall provide resistance to the fire as well as to the passage of smoke and flame equivalent to that of the bulkheads in which the doors are situated. PSCO observed self-closing door to escape trunk pinned open and unable to close completely. 74 SOLAS (20) II-2/9.4.1.1.2

Resolution: Class attended vsl and self-closing mechanism for the escape trunk door was repaired. Door in compliance and resistant to smoke and fire confirmed by class surveyor.

Testing of fire-fighting systems and appliances shall be carried out based on the guidelines developed by the Organization. PSCO's observed crew member attempt to test a flame detector using an open flame cigarette lighter. Crew was unable to produce procedures authorizing open flame lighters as an approval testing device.

SPECIAL CONSIDERATION SHALL BE GIVEN TO THE MALFUNCTION OF THE SOURCES OF WATER PRESSURE. PSCO OBSERVED PORT AND STARBOARD COOLING INTAKE SEA WATER PIPING CORRODED BEYOND REPAIR. THIS PIPING PROVIDES COOLING WATER FOR ALL MACHINERY ONBOARD.
