O2 analyzer was unable to produce less than 16% oxygen, at the time of inspection sample filter found empty, dry and with the gasket o-ring free inside.

ESD trip alarm is not operable in CC for any cargo oil pumps. "Common Trip" was initiated multiple times without external input. ESD trip alarms are unreliable.

A system for the continuous monitoring of the concentration of hydrocarbon gases shall be fitted. Crew tried multiple times to initiate flow fault alarms in the cargo pump room but could not produce alarm in CCR. After tightening fittings, alarms were found operational.
