The owner or managing operator shall make a vessel available for drydock examinations. The following items must be rectified: A) Replace backing plate and hardware on deck fasteners: port side above pontoon chanber 5, and starboard side above pontoon chamber 7. B) Replace broken fitting and repair threads if necessary on #2 port top chanber access. 46 CFR 176.600 (a)

Abandon ship and man overboard drills and training shall be logged. Previous drills have not been logged. Develop plan to log drills.

Fire fighting drills and training shall be conducted or logged. Previous drills have not been logged. Develop plan to log drills.

Vessel must carry adequate and up to date charts. Provide up to date charts.

Remote fuel shut off stations must be marked. Improper marking of fuel shut off valves.

Employer shall provide an EAP for all crew members. Provide posted EAP hotline information and company drug and alcohol statement.

Each pontoon vessel must have a stability letter issued. Unapproved modifications were made to the general arrangements of the vessel's seating arrangements. Provide valid stability requirements. No passengers are to be seated or standing in area of the 2 most aft port benches.

The master of the vessel shall ensure that: The EPIRB's battery is replaced after it is used, or before the date required by FCC regulations in 47 CFR part 80, whichever comes sooner. MI observed the EPIRB onboard had a battery that expired in March of 2023. 46 CFR 185.728(b)

The master of the vessel shall ensure that: The EPIRB test required by paragraph (a) shall be logged or otherwise documented. MI observed there was no log of EPIRB tests. 46 CFR 185.728(c)

The Certificate of Inspection issued to a vessel describes the vessel, the route(s) that it may travel and such other conditions of operations as may be determined by the cognizant [name withheld] discovered the vessel did not carry a copy of the beaching plan dated 8/15/2016 as mandated on the COI. Vessel shall not conduct beaching operations until beaching plan is carried on board. 46 CFR 176.103

Each ring life buoy on a vessel must be stowed in a way that it can be rapidly cast loose. MI discovered the vessel's ring life buoy was stowed in a position where it leaned against the operating station as a potential tripping hazard. 46 CFR 180.70(b)(3)

The master of the vessel shall ensure that each EPIRB must be tested monthly. MI discovered the vessel Master did not keep monthly EPIRB testing logs. 46 CFR 185.728(a)

Licenses for ship stations in the maritime services will normally be issued for a term of ten years from the date of original issuance, or renewal. License authorization expired in May 2021. 47 CFR 80.25(a)

Each life jacket must be marked with Type 1 retroflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. Retroflective material on all life jackets was worn/degraded. Vessel restricted to daytime operations until retroreflective material is replaced. 46 CFR 186.604(h)

The master of the vessel shall ensure that: (a) Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine it is operative; (c) The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. EPIRB monthly checks have not been logged. 46 CFR 185.728

USCG Type A1, A2, B1, or B2 flexible hose may be accepted in accordance with 33 CFR part 183, subpart J. Identified both port and starboard fuel tank vent hoses that were not of the approved type. Replace both hose with appropriate fuel line. 46 CFR 182.720(e)(3)(v)(C)

Ring life buoys shall be of first class workmanship and free from any defects materially affecting their appearance or serviceability. Life buoy color had faded, the foam is cracking, and the grab line frayed upon testing. Replace life buoy. 46 CFR 160.050-4(f)

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. Tested waterlight and found that light was inoperable. Repair or replace with working waterlight. 46 CFR 180.70(d)

A vessel not required to have a power driven fire pump by §181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Identified one fire bucket to have holes in bottom and be inoperable. Replace fire bucket. 46 CFR 181.610

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible…Tide tables, U.S. Coast Pilot or similar publication and Current tables. Replace chart with current modifications. Provide up-to-date tide and current tables and U.S. Coast Pilot or similar publication for applicable waters. 46 CFR 184.420

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Update and replace 04 alcohol testing devices. If kept onboard, store in a temperature controlled environment as per manufacturer’s instructions. 46 CFR 4.06-15(a)(1)

The master of the vessel shall ensure that… the EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Create log for monthly EPIRB testing. Record test information and results in log. 46 CFR185.728(c)

The items contained in first-aid kit shall be as listed in Table 160.041-4(b). Update First Aid kit contents to comply with regulation. 46 CFR 160.041-4(b)

The owner or operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crewmember is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 185.510 of this part. Crew training shall be logged for review by the Coast Guard upon request. The training entry shall include the following information: Date of the training and general description of the training topics. Owner has no record of initial training. Provide updated training record to USCG. - 46CFR185.420

Program VHF-FM radio with MMSI number and GPS input for DSC. 46 CFR 184.502

Replace improper size ring buoy line with 60 feet of 5/16 inch line dark in color or UV resistant. 46 CFR 180.70(d)

Provide light list and coast pilot onboard vessel for operating area. 46 CFR 184.420

Vessel must add adequate hull protection to the pontoons prior to beaching operations in accordance with local CID note policy #8 "Guidelines for Beaching Small Passenger Vessels".

Vessel must obtain a FCC radiotelephone safety certificate. Vessel is restricted to 1000 ft from shore IAW 47 CFR 80.50.
