PSCO observed (02) non-acknowledged alarms on the VDR panel both stating a non-operational float-free VDR. First alarm states "Float-Free Issue; Float-Free - Not Recording". 15109 - The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. PSCO observed 7 overdue, 10 approaching overdue maintenance items as indicated on the MARAD V.5.5.16.26749. Notably, the 2.01 - ME MAK M2S charge air and exhaust gas, charge air cooler was overdue by 53,083 hours. Additionally, there were approximately 40 skipped alarms on the ALMCS display. Alarms of note include water mist fault alarm, fire detection fault alarm failure, and booster unit common alarm (fuel for main engine). Lastly, there was approximately 40 alarms on the exhaust gas cleaning system that include overload analysis input 3 seawater inlet flow, and a warning - wash - water analyzer common alarm. Due to the objective evidence in the above deficiencies, the vessel is not in substantial compliance with relevant conventions. The Captain of the Port questions the adequacy and overall implementation of the vessel SMS under the ISM Code. An external audit conducted by the Flag or RO is required to determine whether the ship is operating in accordance with the ISM Code. 07123 - Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. PSCO observed half the CO2 bottles for the fixed fire suppression system had safety pins in them, and half did not. Vessel [name withheld] was unable to provide sufficient explanation of how the system should be set up. 74 SOLA (20) II-2/14.2.1.2.1.2 30ac

07199 - The purpose of the regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose, the following functional requirements shall be met: means shall be provided to control leaks of flammable liquids; means shall be provided to limit the accumulation of flammable liquids. PSCO observed the paint locker was filled from deck to overhead with open and leaking paint buckets. Additionally, in the purifier room (forward part) there was a drain used to fuel from the HFO/ULSFO day tank into the bilge of the purifier room. 74 SOLAS(20) II-2/4.1.1 and 4.1.2 60ac

11103 - All life-saving appliances and arrangements shall comply with the applicable requirements of the code. Food ration not less than 10,000 KJ for each person the lifeboat is permitted to accommodate shall be kept in airtight packaging. 07119 - Such flexible pipes and end attachments shall be of approved fire-resisting materials of adequate strength and shall be constructed to the satisfaction of the administration. PSCO observed an improvised sounding tube for the sludge tank located starboard side, lowest level of engine near ladder. 74 SOLAS (20) II-2/4.2.2.5.1 17ac

11108 - Each life raft or group of life rafts shall be stowed with a float free arrangement complying with the requirements of paragraph 4.16 of the code so that each floats free and, if inflatable, inflates automatically when ship sinks. PSCO observed port life raft painter was permanently tied to the deck of the ship rendering them inefficient in the event of the ship sinking. 74 SOLAS (20) III/13.4.2 17ac/10c

01299 - Officers in charge of an engineering watch, as defined in the STCW Code, under the direction of the [name withheld] shall be immediately available and on call to attend the machinery spaces and when required shall be physically present in the machinery space during their periods of responsibility. PSCO observed inconsistence between the security watch schedule and shipboard working arrangement (D.4.1(B)). The shipboard working arrangement includes a 2nd engineer. There is not a 2nd engineer onboard the vessel. STCW Reg. VIII/2.2.3 50ab
