PSCO observed excessive quantities of oil in multiple locations in the engine room which pose a significant risk of fire. 1) Oil leaks from #1, #3, & #4 generators. Multiple oil soaked rags and buckets containing 1-2 gallons oil each under generators. Deck underneath generators covered in oil. 2) Significant oil leaks on and under generator fuel oil purifiers. Thick sludge of oil on deck and along purifier space aft bulkhead. 3) Oil soaked lagging on steam pipes & bulkheads, oil covering deck and electrical equipment in space directly below purifier space. 4) Significant accumulation of oil, approx. 25 ft x 25 ft in size, in bilge aft of the main engine. 74 SOLAS (14) I/11

The Company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. 1) Vessel’s safety management system required monthly testing of OWS. Company directed vessel via email in May 2019 to stop using OWS. Vessel has not conducted monthly testing since company email in contradiction of vessel’s SMS manual. 2) Vessel’s SMS required cleaning and identification of leaks per vessel’s SMS form MSOPR-11.15. Excessive oil in engine room indicates that SMS implementation is ineffective. Recommend audit within 30 days. 74 SOLAS (14) IX/3.1 ISM Code A/10.1

The vessel is to prevent the ignition of combustible material and flammable liquids. Means shall be provided to control leaks of flammable liquids. PSCO observed an excessive amount of oil in the bilge pocket around the flywheel. Ship crew indicated the oil was caused by the MDE scrapper ring to seal the crankcase. Due to the location oil can only be cleaned when the MDE is stopped. The MDE must be repaired to prevent excessive amounts of oil from leaking and accumulating around the flywheel when underway.

PSCO observed ship crew cleaning oil around #2 and #4 aux gen where excessive oil was found. The oil was cleaned prior to PSCO departure. PSCO asked crew if the gens were due for overhaul and the crew indicated the request for parts and support was sent to the company in Jan. Both generators must be repaired to prevent excessive oil.
