For the purpose of this regulation, the requirements of the Code shall be treated as mandatory. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. In meeting these requirements, the company should ensure that any non-conformity is reported, with its possible cause, if known. The nature of the below deficiencies provides objective evidence of a serious failure of the implementation of the ISM Code. An external ISM audit report, satisfactory to the Coast Guard, must be submitted prior to clearance of this deficiency and departure.

The purpose of this regulation is to maintain and monitor the effectiveness of the fire safety measures the ship is provided with. For this purpose, the following functional requirements shall be met: fire protection systems and fire-fighting systems and appliances shall be maintained ready for sue. PSCOs observed that the pneumatic tube was disconnected from CO2 bottle E1 D9913. According to the fixed CO2 system diagram the engine room requires 215 CO2 bottles. The disconnected pneumatic tube would have rendered 14 bottles incapable of pneumatic release, leaving only 210 bottles available for pneumatic release to the engine room. According to the ship's maintenance records the fixed cCO2 system was subject to an annual inspection on 17FEB2025 and monthly checks by the crew on 07 occasions since the last annual inspection. The last monthly check was conducted on 19AGU2025 and stated that all discharge piping and pneumatic tubing was not damaged. however, the crew member responsible for conducting the monthly checks of the fixed CO2 system stated that he does not inspect the pneumatic tubing during the monthly checks.
