Oil fuel pipes, which, if damaged, would allow oil to escape from a storage, settling or daily service tank having a capacity of 500 L and above situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned. Two quick-closing pneumatic valves from the No. 2 MDO service tank were blocked open and unable to shut from an outside space.

PSCO observed that quick closing fuel valve on the vessel's number 2 service tank was unable to be fully opened without activating quick closing spring and closing the valve.
