The electrical lights and alarm in the paint locker were not maintained to be considered explosion proof (the cables and fittings were open/plug was missing).

Before the ship leaves port and at all time during the voyage, all life-saving appliances shall be in working order and ready for immediate use. vessel was instructed by Great Lakes-ST Lawrence seaway system authority to stow life rafts not ready for immediate use while transiting locks.
Life rafts were not placed in immediate use after lock transit.
