"In tankers, electrical equipment, cables and wiring shall not be installed in hazardous locations unless it conforms with standards not inferior to those acceptable to the Organization". PSCO witnessed 37 cracked explosion proof lights in the cargo pump room and storage lockers on deck.

"Precautions shall be taken to prevent any oil that may escape under pressure from any pump, filter, or heater from coming into contact with heated surfaces". PSCO observed fuel oil soaked lagging on two of three generators, fuel oil dripping from filter housing. Also found lube oil soaked lagging on return line on main diesel engine.
