The company shall establish procedures to ensure the ship is maintained in accordance with relevant rules and regulations. In meeting these the company shall ensure inspections are held at appropriate intervals. PSCO observed emergency ship service generator required "blackout" maintenance test postponed until 30SEP2025. Ships SMS procedures require written confirmation by the DPA/TSI for any maintenance postponement greater than one time. Last test performed 27MAY2025. Maintenance required monthly. Vessel did not have written confirmation from DPA for postponed maintenance. An internal audit report must be submitted to the Coast Guard and deemed satisfactory prior to clearance of this deficiency.

A second source of energy shall be provided for an additional three starts within 30 min unless manual starting can be demonstrated to be effective. PSCO observed the second starting arrangement unable to start the emergency generator.

Such doors and door frames shall be constructed of steel or other equivalent material. Class "A" fire door on Navigation Bridge, along with class "A" fire door at the bottom of the staircase, had a piece of wood where the lock should be.
