1. The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose means shall be provided to control leaks of flammable liquids. PSCO observed obsessive oil residue in corners of engine room bottom, lagging was saturated with oil. Source of leak is to identified and plan for repair. 74SOLAS (14cons) II-2 Reg 4.1.1

There are diesel fuel leaks on the #3 and #4 cylinder heads of the main diesel engine, creating an ignition hazard in the engine room.

Due to the physical characteristics of the required low-sulphur diesel fuel, the vessel suffered a main diesel engine failure while starting outbound transit and had to be manually restarted and maintained by the engineers [name withheld] to ensure emergency procedures for loss of propulsion due to low sulphur fuels.

The third officer assigned to the vessel does not possess a flag state endorsement for his license, more than 90 days after embarking onboard the vessel. The requirements of the Minimum Safe Manning Certificate are not met without this endorsement
