A tank overflow control system required by this section shall give a visual and audible tank overflow alarm to the ships operator. The 1S cargo tank high level alarm did not give visual or audible alarm in the CCR even though the 1S cargo tank overfill alarm showed alarm and contents of 1S cargo tank at 98% according to radar and manual sounding 5P cargo tank contains over 98% according to radar and manual sounding however the visual and /or audible alarm for 5P high level or overfill alarm in the CCR did not alarm. To assist in casualty investigations, ships when engaged on international voyages shall be fitted with a voyage data recorder(VDR). Vessel has a VDR but does not have an associated manual or referance to identify warning or alarm codes. CITE: SOLAS 74 (14) CH. V Reg.20.1 ACTION: 16ac

The electrical power available shall be sufficient to supply all those services that are essential for safety in an emergency, due regard being paid to such services as may have to be operated simultaneously. The fixed fire detection & fire alarm system shall be so designed & detector so postitioned as to detect rapidly the onset of fire. PSCO discovered plastic bags over smoke detecor and a heat detector in the purifier room in the E/R. CITE: SOLAS 74 (14) CH. II-2 Reg. 7.4.2 ACTION: 10c

PSC Code 1899/30ac: Electrical equipment, cables and wiring shall not be installed in the hazardous locations unless it conforms with standards acceptable to the Organization. Numerous lights located in the hazardous location on deck have exposed wiring, or have water inside the fixtures; so they are no longer safe for use. (74 SOLAS (14 Cons) VII/9 IBC Code 10.1.4

PSC Code 0610/30ac: Chemical tankers and gas carriers carrying cargoes emitting toxic vapours or gases shall carry, in lieu of totally enclosed lifeboats complying with the requirements of section 4.6 of the Code, life-boats with a self-contained air support system complying with the requirements of section 4.8 of the code. During an operational test, there is a uncontrollable leak between the air cylinder and regulator rendering the required pressure unverifiable. Additionally, the vessel does not have the correct adapter necessary for filling the air cylinders. 74 SOLAS (14 Cons)III/31.1.6

PSC Code 2500/30ab: Objective evidence discovered during an expanded ISM exam revealed the following noncomformities: The vessel failed to fully implement the requirements of the ISM Code through their SMS procedures as evident by the following deficiencies indicating that the ship and/or company are not meeting the SMS requirements. Recommend external audit. No. 1 - In meeting these requirements, the company should ensure that: inspections are held at appropriate intervals,any non-conformity is reported, with its possible cause, if known; appropriate corrective action is taken; and records of these activities are maintained. The vessel's crew logged that maintenance had been performed on deck lighting in hazardous areas, lifeboat compressed air system, ballast tank fixed hydrocarbon gas detection system, the nitrogen system, and ODME; however, during an expanded examination those systems on board displayed signs of long lasting, lack of preventative maintenance and/or are inoperable. PSC Code 1840/16ac: Each tank shall be provided with a pressure gauge which indicates the pressure in the vapour space above the cargo. Pressure sensor for No. 6 STBD Cargo Tank displays a communication failure. (MARPOL 73/78 (Cons 17)Annex II/11.1.1 IBC Code 15.14.5)

PSC Code 1840/16ac: Oil tankers of 20,000 tonnes deadweight and above, constructed on or after 1 January 2012, shall be provided with a fixed hydrocarbon gas detection system complying with the Fire Systems Code for measuring hydrocarbon. The vessel's fixed hydrocarbon gas detection system surveyed by class has not been maintained in satisfactory condition since the last survey. (74 SOLAS (14 Cons) II-2/4.5.7.3.1 I/11)

PSC Code 1740/16ac: Subject to the provisions of paragraphs 4 and 5 of regulation 3 of this Annex, oil tankers of 150 gross tonnage and above shall be equipped with an oil discharge monitoring and control system approved approved by the Administration. The vessel's oil discharge monitoring equipment is inoperable. (MARPOL 73/78 (Cons 17)Annex I/31.1)

PSC Code 1899/16ac: Vapour spaces within cargo tanks and, in some cases, spaces surrounding cargo tanks may require to have specially controlled atmospheres. The vessel's nitrogen generation system was inoperable. (MARPOL 73/78 (Cons 17)Annex II/11.1.1 IBC 9.1.1)

PSC Code 1899/16ac: Every ship of 150 gross tonnage and above certified to carry noxious liquid substances in bulk shall carry on board a shipboard marine pollution emergency plan for noxious liquid substances approved by the Administration. The vessel does not possess a shipboard marine pollution emergency plan for noxious liquid substances approved by the Administration. (MARPOL 73/78 (Cons 17) Annex II/17.1)

PSC Code 0140/16ac: An International Certificate of Fitness for the Carriage of Dangerous Chemicals in Bulk shall be issued after an initial or renewal survey to a chemical tanker engaged in international voyages which complies with the relevant provisions of the Code. The vessel's International Certificate of Fitness for the Carriage of Dangerous Chemicals in Bulk references outdated product list being carried on board and used for cargo operations. PSC Code 1840/10ac: A tank overflow-control system required by this section shall: give a visual and audible tank-overflow alarm to the ship's operator. Audible alarm for vessel's high level and overflow detection was not working on deck. (MARPOL 73/78 (Cons 17)Annex II/11.1.1 IBC 9.1.1)

PSC Code 0725/10ac: Ships fitted with bow or stern loading and unloading arrangements shall be provided with one additional foam monitor meeting the requirements of 11.3.7 and one additional foam monitor meeting the requirements of 11.3.7 and one additional applicator meeting the requirements of 11.3.10. The additional monitor shall be located to protect the bow or stern loading and unloading arrangements. The area of the cargo line forward or aft of the cargo area shall be protected by the above- mentioned applicator. 0650 - Lifebuoys - Each lifebuoy shall be marked in black capitals of the Roman alphabet with the name and port of registry of the ship on which it is carried. The life buoys are not marked with the vessel's name. - SOLAS 74/78(2014 cons.) III/7.1.4 - 16c
