The maintenance plan shall include at least the following fire protection systems and fire-fighting systems and appliances, where installed: fire mains, fire pumps and hydrants. Vessel's maintenance plan did not address maintenance, testing, or inspection of the fire main or hydrants. 74SOLAS(04) II-2/14.2.2.3.1

Oil fuel pipes, which, if damaged, would allow oil to escape ... from a tank ... shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside of the space concealed. Shut off valve on the emergency generator fuel tank did not close when operated remotely. 74 SOLAS (20) II-2/2.2.5.1, 17ac

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Numerous flame detectors in the engine room were observed with oil and with material obscuring sensors. 74 SOLAS (20) II-2/14.2.1.2

The condition of the ship and its equipment shall be maintained to confrom with provision of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. The ship's boiler was observed with active steam and water leaks from ruptured piping. Also observed various alarms. 74 SOLAS (20) I/11(a), 17ac

Doors required to be self closing shall not be fitted with hold back hooks. Several self closing fire doors in the engine room were observed to have hold backs without remote release. 74 SOLAS (20) II-2/4.2.1, 17ac

The company and the ship shall comply with the requirements of the ISM Code. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. Objective evidence collected by PSCOs throughout the course of both attendances (10FEB23/15FEB23) has called into question the adequacy/implementation of this vessel's SMS. This evidence includes excessive oil leaks throughout engine room, significant leaks on the fire main, leaking fire hose connections at fire stations, an inop fuel shut-off valve, lack of maintenance on fire detection systems, hold back hooks installed on self closing fire doors, boiler wastage & steam leaks, portable fire extinguishers w/o brackets, a wasted gasket on the sewage tank, inop emergency lighting, improper storage/excessive accumulation of chemical/oil containers. Additionally, crew painted over previously ID'ed soaked lagging rather than effect permanent repairs. based on the overall material condition of the vessel, as substantiated by the objective evidence cited above, an external audit is required. 74SOLAS(20) IX/3.1 ISM CODE 10.1

Steam lines in corridor were exposed without insulation presenting a hazard to persons onboard.
