The Company should establish procedures, plans, and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of personnel. Pilots observed that the pilot ladder they used was incorrectly secured to a pipe rather than designated shackles and padeyes. this created an unsafe condition for personnel embarking the vessel.

Resolved 21 April 2022
Resolution: Class attended and verified proceedures and training. An additonal drill was conducted. Arrangements found satisfactory. See Attached Report.
