Issued 14 September 2017 Resolved
Vsl NOA stated that the vsl was arriving in Ballast and no cargo aboard. It was found that the vsl was loaded with 25K MT of Ammonium Sulfate.
Condition: Improper/Lack of Maintenance
Action required: 17 - Rectify deficiencies prior to departure
Resolved 15 September 2017
Resolution: Update made to ANOA declaring cargo onboard. Vessel submitted corrective actions report to company. Company implemented a fleet wide change to add a designated person ashore review to the eNOA submission process.
