All Life saving appliances should be evaluated tested and approved as provided in 74SOLAS Regulation III/4 and III/5. The #3 Lifeboat was only able to start on one of its battery banks, and the life jackets on the port side were in need of replacement of their retro reflective tape.

Instructions for on-board maintenance of lifesaving appliances complyinjg with the requirements of regulation III/52 should be provided and maintenance should be carried out accordingly. During examination of lifeboats, examiners observed charging cable on lifeboat #4 was pulled from housing. Lifeboat #4 would not start on battery bank #2. Furthermore, air discharge regulator on lifeboat #4 is inoperable. Lifeboat #3 bottle pressure was found at 2500 psi (min pressure is 2800 psi).
Fire Fighting › Structural - A Class Divisions › Fire Doors

Fire resistance of doors should as far as practicable, be equivalent to that of the division in which they are fitted. External doors in superstructures and deck houses should be constructed to at least the class standard and be "self closing" where practicable. During exam, several fire doors in stairway were not closing completely. One door, from galley, was an A60 door that was not fitted with self closing device. Provide master/OIM statement attesting to the proper installation of self closing mechanism and proper operation of all fire doors.
