The vessel experienced a leak in the Main Seawater Cooling System Overboard Valve. The system was rerouted through the high sea chest and the valve was repaired with a soft patch. Provide a repair plan for OCMI approval. Once approved, complete the repair.

The A-60 main engineroom fire door is missing the striker plate and will not properly close and latch in place. Properly repair the A-60 engine room door.

Means shall be provided for automatically giving audible warning of the release of fire-extinguishing medium, the alarm shall operate for the length of time needed to evacuate the space, but in no case less than 20 seconds before the medium is released. The time delay for the engineroom is not marked with the length of time before discharge and the crew was unable to provide documentation about the time delay.
