Each District Commander or Captain of the Port may order a vessel to operate or anchor in the manner directed when: The District Commander or Captain of the Port has determined that such order is justified in the interest of safety by reason of weather, visibility, sea conditions, temporary port congestion, other temporary hazardous circumstances, or the condition of the vessel. Vessel has incurred 02 fires on separate days on the number three cylinder of the main engine. Vessel is required an assist tug rated of 3000 horse power to transit from the sea bouy to inner anchorage. Prior to movement from inner anchorage vessel must contact the USCG marine inspector to ensure the vessel is safe to proceed to berth or move anchorages, which may require an assist tug of 3000 horsepower. 33 CFR 160.111(c)

The construction of doors and door frames in "A" class divisions, with the means of securing them when closed, shall provide resistance to fire as well as to the passage of smoke and flame equivalent to that of the bulkheads in which the door is situated. The A-60 main engineroom fire door is missing the striker plate and will not properly close and latch in place. Properly repair the A-60 engine room door.

Means shall be provided for automatically giving audible warning of the release of fire-extinguishing medium, the alarm shall operate for the length of time needed to evacuate the space, but in no case less than 20 seconds before the medium is released. The time delay for the engineroom is not marked with the length of time before discharge and the crew was unable to provide documentation about the time delay.
