An efficient bilge pumping system shall be provided, capable of pumping from and draining any watertight compartment other than a space permanently appropriated for the carriage of fresh water, water ballast, oil fuel or liquid cargo and for which other efficient means of pumping are provided, under all practical conditions. Cargo hold bilge system inoperable due to failure of suction valve from sea chest to General Service Pump. Make repairs and test. SOLAS-04 II-1/35-1.2.1

The fire safety objectives of this chapter are to prevent the occurrence of fire. Complete bilge & space cleaning of accumulated oil/debris to Engine Room, Sterring Gear room, Bow Thuster space and Incinerator Room. SOLAS-09 II-2/1.1

The emergency electrical power available shall be sufficient to supply all those services that are essential for safety in an emergency for a period of at least 18 hours. Fuel in EDG tank found below the 18 hour mark. Add appropriate amount of fuel to EDG tank.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. 03 cargo hold bilge alarms found damaged or in need of maintenance. Hold #1 Stbd & hold #2 fwd/port- aft/port. Conduct maintenance/repairs/test.

The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids; The following functional requirements shall be met: means shall be provided to control leaks of flammable liquids and to limit the accumulation of flammable vapors. Also, provisions shall be made to prevent overpressure in any oil tank. Air and overflow pipes and relief valves shall discharge to a position where there is no risk of fire or explosion from the emergence of oils and vapor. E/R MDO drain tank is venting vapors into incinerator room through deck drain. Identifiy cause and rectify. Follow all mitigation procedures submitted to USCG Activities Europe until rectified. SOLAS-09 II-2/4.1 & 4.2.2.4 Code 17 changed to 30-days with mitigation procedures in place.

A required fixed fire detection and fire alarm system shall be of an approved type and comply with the Fire Safety Systems Code. Repair fault/damaged smoke detectors found to Loop 14 and test. SOLAS-09 II-2/7.2.2

Cargo hold spaces shall be fitted with either a fixed fire detection and fire alarm system or a sample extraction smoke detection system complying with the requirements of the Fire Safety Systems Code. Correct fault on smoke sampling fire detection system & test. SOLAS-09/19.3.3

EXTERNAL SMC AUDIT- Objective evidence from technical-related deficiencies marked as SMS Related, indicates a serious failure, or lack of effectiveness, of the implementation of the SMS. An SMS external audit focusing on preventative maintenance (deck & engineering) shall be carried out by the RO within the next 30 days. ISM Code Reg 10

All manually operated valves in connection with the bilge pumping arrangements shall be in positions which are accessible under ordinary circumstances. On deck bilge suction reach-rod valve handles to voids and cargo hold emergency overboards on port & Stbd found seized. Conduct repairs & maintenance. SOLAS-04 II-1/35-1.2.5

No electrical equipment shall be installed in paint lockers unless such equipment is: essential for operational purposes; of a type which will not ignite the mixture concerned; appropriate to the space concerned; and appropriately certified for safe usage. Power cable junction into paint store explosion proof fan motor missing proper gasket. Tag out and repair.

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. Cables and wiring shall be installed and supported in such a manner as to avoid chafing or other damage. 02 Reefer electrical boxes on deck found with water tight power cable connections corroded or improperly repaired. Also 50% of plug covers missing watertight seals.

Auxiliary machinery essential for the safety of the ship shall be provided with effective means for its operation and control. SSDG#2 starter air solenoid valve operated starter while generator was in operation. Unknown damage to fly wheel/starting mechanism. Make repirs/test.

Every ship shall be provided with a main steering gear to the satisfaction of the Admin. Steering Gear actuator found with excessive leaking, make proper repairs.

Every steam boiler shall be provided with not less than two safety valves of adequate capacity. Boiler hand relieving gear found not properly relieving. Repair.

Doors fitted in boundary bulkheads between machinery spaces of category A to corridors shall be of A-60 construction, shall be reasonably gastight and self-closing. Engine Room fire door at main deck found with loss of structural integrity. Renew door. SOLAS-09 II-1/9.2.3.3.1 Table 9.5 & II-1/9.4.2.1

As far as practicable, oil fuel lines shall be arranged far apart from hot surfaces, electrical installations or other sources of ignition and shall be screened or otherwise suitably protected to avoid oil spray or oil leakage onto the sources of ignition. Incinerator fuel pump return line found leaking. Tag out until repaired. SOLAS-09 II-2/4.2.2.5.3

Food supply and catering arrangements shall ensure the health and well-being of the crew. It shall be the duty of the master to see that the vessel is in clean and sanitary condition. Clean/sanitize galley. Replace light over cooking area. ILO-147 Convention C068 Article 1

In every ship means shall be provided to prevent overpressure in any part of compressed air systems. Suitable pressure relief arrangements shall be provided for all systems. Provide documentation for safety relief valve bench testing on 04 Unfired Pressure Vessels (start air x2, control air and aux engine starting air).

The following corroded/damaged items identified on deck and in need of maintenance: 1) Stern navigation lights j-box/plugs corroded and detaching. Make temp repairs prior to departure, then full repairs; 2) Rescue boat motor control box watertight cover hinges corroded/detached; 3) On main deck, missing dogging devices on 06 ventilation closures and 02 raised hatch covers; 4) Exterior lights at bow thruster entrance mount corroded/broken; 5) Fire hose box 21 corroded mounts detached at top; 6) Conduct maintenance (grease) on mushroom head ventilation closures and newly installed cargo hold ventilation closures; 7) Bunker station gauges on both sides over-sprayed with paint. ISM Code Reg 10

The following engineering related systems found inoperable or damaged and in need of maintenance: 1) LT cooler leaks at gauges; 2) Temperature sensor for Sludge tank found cut, needs to be renewed; 3) Renew sections of exposed/damaged lagging for Steam piping; 4) Crankcase breather piping over water heaters has a leak, repair; 5) NO.1 generator oil filter is leaking; 6) Stbd E/R fire damper loungers on fiddley air actuator is leaking; 7) All 03 generators oil and fuel line gauge capillary lines are missing securing brackets; 8) Replace missing studs on MSD tank top cover; 9) 03 Salt water pumps found with missing/disconnected/inoperable gauges. ISM Code Reg 10

The arrangement for the storage and distribution of flammable oils employed under pressure shall be as such as to ensure the safety of the ship and persons onboard. Fuel cooling system forward of main engine rigged with hose at pressure relief valve to prevent oil from spilling on deck. 74SOLAS2009 CHII-2 Reg 4.2.4

Measures shall be taken to ensure that the equipment is functioning in a reliable manner and routine tests to ensure continuous reliable operation. Temporary wiring and heater installed on HFO drain line. 74SOLAS2009 II-1 Reg 46.2

The main inlets and outlets of all ventilation systems shall be capable of being closed from outside the space being ventilated. Multiple ventilation closures found wasted. 74SOLAS2009 CHII-2 5.1.2.1.1

Doors for watertight integrity of internal openings shall be provided with indicators to show whether the doors are open or closed. Watertight closure indicator found tied back disabling the sensor. 74SOLS2009 CH II-1 Reg 13-1.2

Following installation of a BWMS, the master, owner, operator, agent, or person in charge of the vessel must maintain the BWMS in accordance with all manufacturer specifications. Automatic valve to ballast water overboard not operating as designed. To be corrected next dry-dock.

Electrical installations shall be such that maintaining the ship in normal operational condition will be ensured. Lighting at gangway found operating not as designed. 74SOLAS2009 II-1 Reg 40.1.1

Guard rails or bulwarks shall be fitted around all exposed decks. Various rails and courses throughout the vessel's open decks were found to be significantly wasted/damaged and were marked for repair. All identified railings/courses are to be repaired at next dry docking. ICLL(05)25.(2)

Performance of crew members assigned fire-fighting duties shall be periodically evaluated by conducting on-board training and drills to identify areas in need of improvement, to ensure competency in fire-fighting skills is maintained, and to ensure the operational readiness of the fire-fighting organization. USCG Marine Inspectors (MIs) identified the following during fire drills conducted during annual inspection: Fire-fighters and team members assisting during dress out failed to properly don equipment. One fire-fighter donned SCBA upside down, did not use lower straps, multiple fire-fighters did not strap helmets (which fell off during drill), skin was exposed around flash hoods/collars and one flash hood was put on over the mask. During drills one SCBA bottle expelled all air for an unknown reason (investigate failure) and a second SCBA regulator abnormally squealing. Based on location of drill, a second set of fire-fighters outfits were unavailable and due to the SCBA loss of air, 01 crewmember entered into space to attack fire without a fire line or radio. Fire-fighter attempted to fight simulated fire with portable fire extinguishers unsupported. Communications with bridge were lacking. Crew shall conduct additional fire drill in the presence of USCG Marine Inspectors prior to departure. 74SOLAS14/II-2/15.2.2.3

Smoke detector "MD.15 BOSUN1" in the Bosun Stores failed testing. Repair/replace as necessary and conduct testing to satisfaction of attending USCG marine inspector. 74SOLAS14/II-2/7.1.1

Fixed fire detection and fire alarm systems shall be suitable for the nature of the space, fire growth potential and potential generation of smoke and gases. Smoke detector found inoperable due to the welder’s disconnect timer switch failing to reset in the engine room workshop. Second test satisfactory after adjusting disconnect switch. 74SOLAS14/II-2/7.1.1

A minimum of two two-way portable radiotelephone apparatus for each fire party for firefighters communication shall be carried on board. Those two-way portable radiotelephone apparatus shall be of an explosion-proof type or intrinsically safe. Ships constructed before 1 July 2014 shall comply with the requirements of this paragraph not later than the first survey after 1 July 2018. Vessel does not have any of the required radios required by this regulation. 74SOLAS14/II-2/11.10.4

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Rescue boat's helm was inoperable. Repair helm and demonstrate operation to attending USCG marine inspectors. 74SOLAS14/III/20.2

Every steam pipe and every fitting connected thereto through which steam may pass shall be so designed, constructed and installed as to withstand the maximum working stresses to which it may be subjected. Boiler Feed water piping / flange connection found with water/steam leak. 74SOLAS14/II-1/33.1

Fire protection systems and appliances shall be maintained ready for use. The manual fire alarm outside the EDG space was missing faceplate. Replace the missing faceplate. 74SOLAS14/II-2/14.1.1

Satisfactory means for safe passage required by Regulation 25-1 (in the form of guard rails) shall be provided for the protection of the crew for spaces used in the essential operation of the ship. Railing/courses IWO aft HPU single point crane rails wasted, and Ports side mooring line storage reels wasted. Provide a temporary repair until permanent repairs may be completed. ICLL(05)25.(4)

The certificates of inspection issued to US vessels, describe the vessel, the route, the minimum manning requirements, the safety equipment and appliances required to be on board, the total number of persons that may be carried, the names of the owners and operators, and the period of validity. Additionally, vessels must undergo an annual inspection within the 3 months before or after the anniversary date. Due to the extenuating circumstances, all existing information on the current COI of the SLNC MAGOTHY is certified to be true. This deficiency shall serve as approval by the US Flag to extend the due date for the annual inspection until 30APR2021 or upon entry to first U.S. port, which ever comes first. 46 CFR 2.01-5 & 46 CFR 91.27-1

Crew Members should be trained to be familiar with the arrangements of the ship as well as the operation of any fire-related systems. Instructions for labeling of controls for Fixed CO2 Systems was confusing and not quickly identified by crew member. Label & identify components that are necessary to operation of Fixed CO2 systems the corresponds with Instructions.

Means for securing & maintaining weather tightness shall be to the satisfaction of administration. Cotter pins for multiple dogs & hinges on main deck were missing.

Auxiliary steering system shall be capable of moving rudder from 15 degrees on one side to 15 degrees on the other side. No.1 steering pump auxiliary controller switch broke during testing. Replace and show proper operation.

The machinery shall be of a design and construction adequate for its intended service. Vessel experienced loss of power and loss of propulsion due to computer program failure in automation system. DNV-GL to make assessment and certify proper operation of subject machinery prior to getting underway from current port.

In a ship in which oil fuel is used, the arrangements for the storage, distribution, and utilization of the oil fuel shall be such as to ensure the safety of the ship and persons on board. The number 2 diesel oil storage tank was found with an approximate 200 mm long by 1 mm wide vertical fracture at frame 14. The fracture is located approximately 1.4 meters above the bottom of the tank on the inboard side of the tank. Make permanent repairs to the number 2 diesel oil tank prior to storing diesel oil inside the tank and contact the nearest USCG OCMI for an examination of repairs when repairs are ready to be completed.

Fire protection systems and firefighting systems and appliances shall be maintained ready for use. Engine room and emergency generator ventilation louvers do not properly close. Demonstrate proper operation of all louvers.

The company should establish procedures for key shipboard operations concerning the safety of personnel. The ballast water control system is not operational and not properly locked out/tagged out.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations. The CO2 vent fan is not operable. Demonstrate proper operation.

Terminations in all conductors shall be so made as to retain the original properties of the cable. The wires for the flame detector above the #1 generator are exposed. Properly secure the wires.

Electrical installations shall be such that the safety of crew from electrical hazards will be ensured. Old batteries are currently stored in the emergency generator room. Remove all excess batteries.

Stairways and lift trucks which penetrate more than a single deck shall be protected by self closing doors. 3 self closing doors did not completely close.

Light switches thru out crew quarters were found to be either inoperable or in a faulty condition posing an electrical hazard to crew members.

Proper sanitary facilities/showers shall be provided in all crew staterooms.

All embarkation lights IWO life saving stations shall be made operable locally and switches properly mounted where needed.

Excessive oil accumulations were found thru out engine room bilges.

Main engine found to be leaking steady stream of fuel while running on diesel and significant lube oil leaks IWO fwd engine seals were noted causing accumulations of oil in bilges.

A-class fire door between engine room and steering compartment found to be damaged and not closing properly.

Failure to maintain an accurate record log book. Please provide proof of remedial training and extend of audit results.

Rocker arm bolt on #4 cylinder broke. Provide signed report of repair attesting to the renewal of the bolt and satisfactory engine operation.

Vessels NS5 system was OOC and vessel did not have a secondary means to record machinery maintenance.

Rams seal are currently leaking during operation

Replace cover on LT cooling Temp regulating vlalve.

Repair FWD mast head light and port running light.

Repair bracket for ring buoy light.

Repair hydraulic leak in fwd crane.

#2 SSDG shutdown during normal operation. Troubleshot by manufacture service tech led to #9 bearing within generator. Require proper repair and full operational test report from class surveyor.

The OWS on the vsl in not operational, there is no record of running the OWS dated back to July 26, 2015 in the ORB. Make repairs to the OWS and prove operational test

The oil content meter (OCM) must have a current calibration certificate. The current OCM certificate expired July 14, 2010. Provide current calibrated OCM or have current one calibrated.

During review of bunker delivery notes, samples, and change over procedures it was noted that DMA of .46% was received on 30SEP15 and on 17AUG15 Fuel (gasoil) having a .33% content was refused. Provide written acceptance procedures per SMS in regards to when to accept/decline low sulfur fuel for use in ECA.
