USCG Marine Inspectors (MIs) identified the following during fire drills conducted during annual inspection: Fire-fighters and team members assisting during dress out failed to properly don equipment. One fire-fighter donned SCBA upside down, did not use lower straps, multiple fire-fighters did not strap helmets (which fell off during drill), skin was exposed around flash hoods/collars and one flash hood was put on over the mask. During drills one SCBA bottle expelled all air for an unknown reason (investigate failure) and a second SCBA regulator abnormally squealing. Based on location of drill, a second set of fire-fighters outfits were unavailable and due to the SCBA loss of air, 01 crewmember entered into space to attack fire without a fire line or radio. Fire-fighter attempted to fight simulated fire with portable fire extinguishers unsupported. Communications with bridge were lacking.
